South Sefton Primary Care Network is a large PCN covering the four localities of South Sefton, with a population of just over 155,000 patients.


PCNs are groups of GP practices working with a wide range of partners across the health and care system to improve the health of their population.

Key to this purpose is strengthening general practice provision through working together, where it makes sense to do so and working as part of integrated care teams which work collaboratively for and with local people in each locality.

Job Purpose


This is an exciting opportunity to work with South Sefton Primary Care Network, our GP Federation, South Sefton Primary Health Care Limited and the 28 constituent practices to develop Sefton Access Service.


The post holder will be a caring, compassionate and committed experienced clinician who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the primary care setting.

The post holder will commit to developing critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the Sefton Access Service team to meet the needs of patients, supporting the delivery of policy and procedures, and developing clinical leadership skills


This trainee ACP will work as part of the overall Sefton Access Service primarily serving patients with acute and minor illnesses.

Some evening and weekend work may be required in line with the provision of wider Sefton Access Service.

Job Responsibilities

Clinical Practice

  • Develop competence to assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Develop competence to clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Develop competence to proactively identify, diagnose and manage treatment plans for patients at risk of developing a longterm condition (as appropriate).
  • Develop competence to diagnose and manage both acute and chronic conditions, integrating both drug
- and non-drug-based treatment methods into a management plan.

  • Become competent in prescribing and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidencebased practice and national and practice protocols, and within scope of practice.
  • Develop competence to work with patients in order to support compliance with and adherence to prescribed treatments
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

  • Learn to prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Become competent to advise, support and administer vaccinations for patients travelling abroad, where appropriate
  • Develop competence to promote and deliver evidencebased care for patients presenting with aural conditions
  • Learn to meet the needs of patients presenting for opportunistic wound care
  • Develop competence to undertake minor surgery as appropriate and within competence.
Communication

  • Learn to utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
