The Role


The purpose of this role is to provide expert professional advice as an autonomous Nurse/ACP Consultant to patients, carers and colleagues in the specialist field of frailty.

The post holder will actively participate in and undertake research and will provide education and training to other staff and students in the specialism of frailty.

The post holder will ensure the maintenance of clinical excellence within frailty, working autonomously as a senior clinical decision maker primarily in the acute frailty service within same day emergency care (SDEC) and the frailty virtual ward, linking closely with community partners, NWAS and the primary care networks, supporting admission avoidance and clinically supervising the team of frailty ACPs.


What you'll be responsible for

  • Have expert knowledge of the specialist clinical area
  • Function as a competent autonomous clinical practitioner assessing and managing complex patients and their families as an integral part of the team
  • Work as part of the medical or specialist team to provide cover for patients as part of the appropriate rota within the specialty. This may include rotation to nights and seven day working
  • Be active in initiating, undertaking and participating in audit and research projects relevant to the specialty
  • Be accountable for their actions and act in accordance with the NMC Code of Professional Conduct/ Health Care Professionals Council Code of Conduct fulfilling the pillars of advanced practice and working autonomously in the specialist field, supporting other staff including advanced practitioners
  • Will act in accordance with the local Trust and statutory guidelines and policies

You'll have the following qualification/experience:


To be professionally and legally responsible and accountable for all aspects of own work, ensuring a high standard of clinical care is delivered.

To act as the patient's advocate at all times.


To ensure appropriate supervision and clinical support is sought when required and always work within own scope of professional practice and in accordance with the level of developing competence.


To have and maintain the skills and knowledge to accept clinical responsibility for a diverse and often complex caseload of patients, and to develop the ability to organise this efficiently and effectively with regard to clinical priorities and use of time and to respond appropriately to patients who are or become acutely unwell and deteriorate, escalating and intervening appropriately.

To prescribe medications from an agreed formulary and ensure appropriate medication is prescribed by means of transcribing.


To receive referrals from and make referrals to consultants, GPs and other healthcare professions, to facilitate appropriate reviews of patients.


To actively promote a clinical environment that is conducive to high quality patient care and high standards of service delivery.


Uses specialist knowledge, leadership and expertise to assess, plan and implement care interventions for patients requiring complex care and monitoring which may include patients with a poor or unpredictable prognosis.
