- Collaborating with multi-disciplinary partners to assess individual referrals and determine the most suitable support pathway in compliance with the Care Act.
- Embracing innovative solutions that consider community resources to facilitate safe discharges from healthcare facilities.
- Enhancing the knowledge of health colleagues through training and promoting an understanding of Prevention and Early Intervention principles.
Requirements:
- In-depth knowledge of relevant legislation to effectively engage with health colleagues.
- Experience in needs assessment and support provision within the health system.
- Strong communication skills, both verbal and written, with the ability to work independently.
- Proficient in literacy, numeracy, and administrative tasks.
- Excellent planning and organizational abilities with a flexible and resilient approach to deadlines.
Additional Skills:
- Familiarity with the Care Act and Mental Capacity Act.
- Background in multi-disciplinary collaboration to optimize individual outcomes.
