Telford Mind have a fantastic new opportunity for a full time Health Inequalities Link Worker who is a highly motivated and efficient individual with the ability to work on their own but part of the operational team with good communication and interpersonal skills.

Hours of Work: 35 hrs/week. Monday to Friday between the hours of 9.00 am pm (flexible working is required to suit the needs of the service).

Job Purpose:


The Health Inequalities Link Worker will be required to develop a service, that improves accessibility and engagement with primary and secondary and health services for the homeless population.

The service will support individuals to take control of their wellbeing and live happier, healthier lives. There will be an emphasis on mental health, supporting people with their social, emotional health and wellbeing needs.

The Health Inequalities Link Worker will coordinate and make links with the service providers to help individuals access services.

The project intends to empower the individuals by improving their decision-making capacity and having a say in the policy and strategy.


You will act as a 'First Point of Contact' prior to individuals being identified as homeless and introduce them to the homelessness buddies.

The Health Inequalities Link Worker will ensure that the most appropriate community services are involved with the individual in order to ensure they are effectively supported within the community.

This role will seek to understand the needs of the individual in order to effectively support them to access the most appropriate support for them.

This service cannot operate in isolation and that effective partnership working is needed to enhance the individual's experience.


You will be required to attend the relevant Forums in order to meet the needs of the service and service users.

You will adopt a multi-disciplinary team approach to achieve positive, sustainable outcomes for our service users.


You will also be required to build, develop, coordinate and support a workforce of volunteers to promote, develop and deliver a homelessness outreach buddy service to individuals across Telford and Wrekin to compliment this project.

You will need experience and qualifications relevant to the role and a desire to make a difference. You will need access to your own vehicle and a full clean driving licence with business insurance.

Main Duties

Development

  • Raise awareness and understanding of the service amongst the relevant services and Local Authority.
  • Develop referral processes which make the service as accessible as possible to people who could benefit
  • Work in partnership with relevant colleagues to meet people's needs, including ensuring staff get feedback
  • Work in partnership with relevant colleagues to understand their people's needs, using local knowledge and data to target specific cohorts of people and ensure service data is captured
  • To recruit, develop and coordinate a workforce of volunteers.

Delivery

  • Identify suitable referrals and develop a personalised action plan of the individual's desired outcomes.
  • To work with identified homeless individuals, who reside in Telford & Wrekin
  • Proactively engage people into the service using a variety of approaches, including receiving referrals and contacting people directly to discuss the service
  • Deliver a service that is flexible, person centred, focuses on people's strengths and personal goals and provides them with choice
  • Coproduce a personalised support plan with the service user to improve their health and wellbeing, introducing or reconnecting people to community groups and statutory services as appropriate
  • Deliver a range of direct interventions depending on individual needs including;
  • Emotional support including support to develop effective coping strategies (working therapeutically)
  • Support to better manage or improve physical health
  • Support to regain or maintain relationships with friends and family
  • Support to develop more confidence and a greater ability to gain choice, control and responsibility in their lives
