The Care coordinator is primarily responsible for supporting the PCN team in the collaborative delivery of services and support, based mostly at our premises at Ethelbert Road Surgery.

The focus of the role will be on co-ordinating care for patients with suspected or diagnosed cancer as well as cancer screening services, however, as the role evolves it could include other areas.

The care coordinator may be required to deal with patients and, if appropriate, their carers, before or after the patient's consultation with a clinician or other healthcare professional.

You might also be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.


It is important to note that responsibilities will be varied and dependent upon CIH needs and this is not designed to be an exhaustive list.

The following are the core responsibilities of the role:
General - Provide support to patients with new and ongoing cancer diagnoses. - Provide administrative support for reports and data collation as and when required, using EMIS search and reporting tools.

- Working closely with the existing CIH team to provide support for the PCN DES and IIF targets across Canterbury.


  • Support with increasing the uptake of screening services in the local area
  • Working with other help set up collaborative services for Canterbury patients.
  • To assist the Primary Care Networks in delivering improvements to the following areas: o Engaging with national cancer screening campaigns and implementing to the local area o Ensuring robust and supportive referral practices are in place for patients suspected of having cancer; including use of guidelines, professional development and safety netting of those referred o Form productive and supportive relationships with practice staff who have the skills, knowledge and remit to contribute to this work. e.g. GP practice non clinical cancer champions, social prescribers and secretaries.
