JOB SUMMARY:

To support the Clinical & Operational Service Leads for Community Integrated Neurology and Stroke Services (CINSS) in the development and provision of effective, efficient and high quality services.

In collaboration with the Clinical/Operational Service Leads, work to the strategic direction of the CHS Directorate and the modernisation of Community Integrated Neurology and Stroke Services (CINSS).

To lead on the development and operational implementation of national clinical guidelines, Clinical Pathways and Training/Education within the service.

To work as part of a dynamic team to ensure safe, effective, and high-quality care pathways are in place for all patients within the service, including case management responsibility for the patients on the ABI pathway.

To work with acute health services and slow stream rehabilitation placement providers tofacilitate/coordinate personalised care and rehabilitation for patients whilst on the ABI slow stream pathway.

To work closely with the CINSS Clinical Service Leads to organise, develop, and maintain an efficient and effective ABI funded pathway service to LLR patients in the post-acute community setting.

To have responsibility for monitoring the implementation of ABI clinical guidelines and personalisation of rehabilitation programs within provider services.

To have clinical responsibility for a designated specialised caseload across the service

CLINICAL:

To work as an autonomous clinical practitioner, undertake highly skilled/developed OT/Physio assessments to determine, develop and implement individual care/treatment plans for patients, some of whom may have complex needs.

To work as an autonomous clinical practitioner without direct supervision but with regular access to professional advice and support from the Clinical Service Leads.

To demonstrate highly developed dexterity, co-ordination and palpation skills for assessment, manual treatment and therapeutic handling of patients.


To case manage, coordinate, and facilitate individual patient journeys on the ABI pathway as required to ensure high quality service and care planning outcomes are achieved.

To facilitate a coordinated approach from referral onto the ABI funded pathway, individual care/support needs, rehabilitation, and discharge planning from the ABI pathway, including proactive robust care packages as appropriate to individual patient needs.

To undertake measurement and evaluation of own and others work through the use of evidence based projects, audits and outcome measures, either individually or in co-operation with others for the service.

To accept clinical lead and case management responsibility for a designated caseload of specialised patients across the service, and to organise this effectively and efficiently with regard to clinical priorities and use of time.

To manage clinical risk within own patient caseload when working in the community setting as a lone practitioner.

To interpret and analyse clinical and non-clinical information to form an accurate assessment and prognosis across a wide range of conditions, to recommend the best course of intervention and to develop comprehensive rehabilitation plans.

To assess capacity, gain valid & effective consent and work within a legal framework with patients who lack capacity to consent to treatment.

To evaluate patient progress, reassess and alter treatment programmes if necessary.

To ensure the appropriateness of the placement for patients on the ABI pathway.

To interpret and analyse complex clinical and non-clinical information to form an accurate assessment and prognosis across a wide range of needs associated to patients referred to the service, to facilitate, coordinate and monitor the best course of intervention from the service and/or the ABI service providers.

To evaluate patient progress, reassess and suggest/facilitate and/or implement altered treatment programmes as required for patients within the service. To take action in respect of Safeguarding Policies and Procedures as required.


To be professionally and legally accountable for all aspects of own work, including the management of patients in the post holders care.

To maintain accurate, comprehensive, and up to date records/documentation through a paper and/or computerised system in line with agreed standards.

To communicate the progress reports including updates on assessment and treatment results and clinical outcomes via the appropriate reporting lines verbally or in the form of reports and letters, this will include representing the service or patient at multi-disciplinary team meetings where patient care, patient progress and discharge may be discussed.
