Physiotherapist - ABI Rehabilitation Specialist

We are seeking a highly skilled and dedicated Occupational Therapist, Physiotherapist, or Speech and Language Therapist to join our Acquired Brain Injury (ABI) case management service. As a key member of our team, you will play a vital role in ensuring the safe, effective, and high-quality care pathways are in place for patients with ABI or strokes.
Key Responsibilities- Work collaboratively with our dynamic team to develop and implement personalized care and rehabilitation plans for patients on the ABI slow stream pathway.
- Coordinate with acute health services and rehabilitation providers to facilitate timely discharges and ensure seamless transitions to community care.
- Collaborate with our Clinical Service Leads to maintain and improve the efficiency and effectiveness of our ABI funded pathway service.
- Monitor the implementation of ABI clinical guidelines and develop personalized rehabilitation programs within provider services.
- Utilize your clinical expertise to deliver high standards of practice and contribute to the development of our team's clinical and professional skills.
- Take clinical responsibility for a designated caseload across the service, working flexibly to support the needs of our patients and team.
