Patient Flow Coordinator

The Patient Flow team has day to day operational responsibility for providing the safe management of patient flow pathways across the hospital.

You will be involved in supporting the smooth running of all aspects of capacity management and help to ensure a seamless patient journey, to enable the Trust to meet the key strategic and operational objectives, ensuring local and national standards are achieved.

Intervention will include actions to prevent delays, for example chasing results, transport and discharge outcomes. The post-holder may be required to assess the patient's wellbeing using acquired experience and clinical knowledge, prior to handover.


The Flow Co-ordinator may be either a clinical or administrative clinical role with expert knowledge of patient pathways within the NHS, who can support the senior nursing team in ensuring the targets are met, escalating when required.

You will be required to support the team in ensuring the Discharge Lounge is used to its full potential.

MAIN DUTIES AND RESPONSIBILITIES

  • To work within the multi-disciplinary team to provide real time information to Senior Sister for Flow for bed states and patient flow.
  • Liaise with the discharge lounge around capacity and any issues with patient flow.
  • Laise with case managers to ensure that all patient information is accurate
  • Liaise with other Trust departments and specialities to minimise delays when patients are referred.
  • To utilise the skills, knowledge and expertise of all members of the MDT in order to facilitate the safe timely and effective discharge of patients.
  • Ensure patients flow appropriately through their allocated clinical area, for example maximising use of the discharge lounge and liaising with medical colleagues
  • Ensure patients receive their medications prior to leaving the department by working closely with pharmacy colleagues
  • Chase up delays with patient transport so patients are able to go home as quickly as possible
  • Work with Radiology colleagues to ensure imaging is prioritised and in order to avoid delays in discharge.
  • To undertake weekly length of stay review for all inpatients 7 days, 14 days and 21 days.
  • Liaise closely with colleagues to prioritise and plan own workload, reporting to the Senior Sister for Patient Flow
  • Liaise with ward managers to ensure SAFER Boards, EDDs and Medically Fit are up to date
  • Help patients, their relatives or carers with queries or complaints, only escalating to a senior member of staff if this cannot be resolved or is of a serious nature
  • Ensure colleagues and other departments are aware of potential delays and where there are blockages in the patient pathway, liaise closely with Site team and wards
  • Will require moving and handling, for example supporting patients who may not be able to mobilise, pushing wheelchairs

LEADERSHIP RESPONSIBILITIES

  • Support patient flow with national standards
  • Occasionally participate in audit and research as directed by a senior team member
  • Attend daily social service meetings, as required to represent their areas, updating on key issues and escalating any issues

COMMUNICATION

  • Communicate efficiently and in a timely manner to ensure any potential problems are escalated
  • Communicate with patients, their relatives and carers to ensure they are aware of what is happening
  • To communicate immediately any potential problems regarding the patients discharge to the appropriate person
  • Communicate with patients where there may be barriers to communication such as learning disabilities, always maintaining empathy and compassion with the patient.
  • The post holder will work closely with the clinical multi-disciplinary teams to ensure good communication between staff, patients and their relatives in regards to prompt and safe discharges.
  • Ensure all communication is presented appropriately to the different recipients, according to levels of understanding, type of communication being imparted and possible barriers such as language, culture, understanding or physical or mental health conditions.
