About the Role

We are seeking a highly skilled and compassionate Healthcare Assistant to join our team at Modality Partnership. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and supporting the health and wellbeing of our patients.

Key Responsibilities
  • Assess patient needs and provide pre-screening activities and healthcare for chronic diseases
  • Support patients with health education, advice, and support
  • Implement requirements in line with patient needs, community, or other settings
  • Maintain clinically accurate records during clinics
Requirements
  • Basic IT knowledge and skills
  • Demonstrates understanding of person-centred care
  • Willing to develop clinical skills and competency relevant to the role
  • Working in a primary or secondary care setting
  • Recent previous experience in direct patient care
