Key Responsibilities- Assess patient needs and provide pre-screening activities and healthcare for chronic diseases
- Support the wider nursing team to promote patient health and wellbeing
- Conduct health checks, including blood pressure and ECG monitoring, and take blood samples
- Provide health and wellbeing advice and promotion
- Work collaboratively with the general practice team to meet patient needs
- Basic IT knowledge and skills
- Demonstrates understanding of person-centred care
- Willing to develop clinical skills and competency relevant to the role
- Working in a primary or secondary care setting
- Recent previous experience in direct patient care
