Key Responsibilities- Identify life roles and activities that are meaningful to patients and plan care to address barriers to perform these activities.
- Work closely with the multi-disciplinary team to provide comprehensive care to patients.
- Develop and implement individualized care plans that promote independence and quality of life.
- Conduct thorough assessments and provide evidence-based interventions to address physical, emotional, and social needs.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Highly specialist knowledge and application of therapy assessments and interventions using complex level clinical reasoning.
- Experience of evidence-based practice and outcome measures.
- Motivational skills and goal setting for personalized care.
- Ability to work autonomously and set own and others' priorities.
- Excellent advanced communication skills.
- Evidence of relevant postgraduate courses.
- Diploma/BSC (Hons) in Occupational Therapy (OT).
- Registered with the Health and Care Professionals Council.
