We have exciting opportunities for Band 5 Nurses to work across our PCN Sites within Surrey Downs Health & Care. If you are passionate about caring for people and making a difference in your community, we would love to hear from you.

We are looking for nursing staff from all backgrounds, including newly qualified individuals and those with more experience, who are seeking a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that it brings.

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation, and evaluation of holistic care packages.

Please note these roles require post holders to hold a current and valid driving license enabling them to drive in the UK and have access to a vehicle for work purposes.

We have our own Learning & Development Department that offers a range of courses and development plans to help you grow your career.

Main Duties of the Job

PCN - Primary Care Network

It is a network formed from GP practice populations working in partnership with others to provide health and care to local communities.

What will PCNs do?

  • Provide ongoing care and support
  • Deploy the most appropriate staff member(s) in a coordinated and timely way to best utilize community resources
  • Reduce the risk of admission for patients
  • Support people in their homes, people in care homes, and through in-reach into hospitals where necessary
  • Work closely with the community hospitals and specialist teams

Each PCN has a community-focused triumvirate of Lead GP/Community, Lead Clinician, and Operational Manager. Specialist services and community hospitals work closely with the PCNs to ensure a whole system approach to working across Surrey Downs.

Dorking

(Medwyn Centre, Reigate Road, Dorking RH4 1SD)

Dorking community nursing team covers central Dorking & the beautiful surrounding villages of Surrey. Dorking can be described as a charming market town, surrounded by the beautiful Surrey Hills. While working in the community, you can catch a glimpse of the wooded hills and open landscape. As you drive through the streets of Dorking, you can experience the historic streets of the town. Dorking is close to the M25 with good access from Guilford and London, making Dorking an excellent place to work. Flexible hours can be considered - during the day, we cover 8 and have a twilight team. We work closely with the GP practices, Matrons, physio, hospices, and Dorking community hospital to deliver interventions such as blood tests, wound care right through to high-level complex care with the aim of avoiding hospital admissions. Leatherhead Primary Care Network

(Leatherhead Hospital, Poplar Road, Leatherhead, KT22 8SD)

Leatherhead PCN team will be undergoing transformation as part of SDHC business plan. As part of this transformation, successful applicants may be required to work from a different base in the future and will be part of the consultation process for this.
