Job summary

This is an exciting opportunityfor a band 5 community staff nurse to join an expanding Responsive service teamwhere acutely unwell patients can be cared for in their own homes. This rolewould be suitable for both experienced and newly qualified nurses as part ofour Preceptorship Programme.

Responsive services allowpatients to receive the care they need at home safely and conveniently ratherthan being in hospital. The multi-disciplinary service works 7 days a week from8am 8pm.

You will work in a caring andsupportive environment under the supervision of the virtual ward matron closelywith our urgent community response, intermediate care and virtual ward teams.

You will support the individualswithin their home environment with all aspects of care including daily living,providing person centred care and promoting health and independence throughawareness raising and care navigation to other professional as appropriate.

You will need to be able to work autonomously and as part of a team. Tosupport you within your role, we have an excellent learning and development tohelp further develop your clinical skills.

You must be a licensed car driver and have access to a car for this post.

Job responsibilities

Thepost holder will work under the direction of the virtual ward charge nurse andin collaboration with Primary Care and Secondary Care Colleagues, UrgentCommunity Response, Intermediate Care (Responsive Services), RespiratorySpecialist Team, Community Nursing and other Specialist Health Care Teamsacross First Community, statutory and voluntary agencies to deliver excellentstandards of patient care for patients who are acutely unwell requiring supportto avoid admission to the acute hospital. . Providing practical care, educationand training for patients and carers admitted to the virtual ward andresponsive services.
