We are looking for a dynamic and forward-thinking experienced Stroke SpecialistNurse with a passion for delivering high quality, patient-centred first-rate care. The successfulcandidate will be focussed on the ongoing development of our dedicated stroke service.

Centralto the role are strong communication and organisational skills, provenspecialist clinical skills in stroke management and rehabilitation as well asexperience of line management and clinical supervision.

As the teamworks across multiple sites to deliver patient care, candidates must have afull driving license and access to a car for work purposes.

Main duties of the job

The postholder is responsible for working collaboratively within our Community Strokeand Neuro Rehabilitation Team (CSNRT) and Neuro Specialist Practitioner Team intheprovision, development and co-ordination of the community stroke services.

The post holder will need to establishand maintain links with healthcare professionals, managers and support workerswithin both primary and secondary care to support patients on discharge fromhospital as well as implementing care pathways to support patients to live wellfollowing a stroke, leading on the delivery of six-month post stroke reviews.

The post holder will be responsiblefor a defined patient caseload, initiating treatment plans, risk factormanagement, chronic disease management, providing nursing care, support andeducation to patients and their carers. They will apply their expertise toenable patients post stroke to receive advanced specialist care in their ownhome and in the community.

Job responsibilities

The CommunityStroke Specialist Nurse will act as a role model, providing high standards ofevidenced based holistic care in adherence to the Nursing Midwifery Councils(NMC) The Code. They will ensure that the care and specialist support theyprovide to stroke patients is aligned with current national stroke guidance,NICE guidance, the Stroke Service Specification and other relevant qualitystandards.

The post holderwill apply the advanced knowledge of stroke to develop and provide specialisteducation acting as an expert resource for advice for healthcare professionals,patients and carers.

Person Specification

Qualifications

Essential

Current NMC Registration Education to Degree level or other relevant qualifying programme Evidence of further relevant post-registration courses Adult Physical Assessment qualification Teaching and Assessing Qualification. Independent Prescribing Qualification (or prepared to undertake and has the pre requisites to do so).

Desirable

MSC or MA or working towards Leadership and Management Course Membership of specialist interest group Counselling qualification

Motivated and committed to delivery of patient centred care. Enthusiasm to learn and develop further expertise in this field.

Experience

Essential

Minimum 5 years post registration experience Minimum of two years specialist stroke nursing experience Clinical experience of working in Specialist Stroke Services Evidence of involvement in service development projects. Experience of leading a team
