We are recruiting band 4 NurseAssociate (NMC registration) to join our expanding community teams in East Surrey,based in Redhill.

You will bepart of a highly motivated and supportive district nursing team delivering safeand effective NHS care to patients in the community.

In return, weoffer a comprehensive induction programme, clinical supervision, support withcontinuing professional development and revalidation.

You will besupported by the Clinical Team Lead for the Community Network and your linemanager to develop your skills and will work independently to supportdevelopment of the service and implement evidence-based practice.

You will providenursing care for a wide range of conditions to people in their own home andresidential settings, and you will be supported by a manager, team and mentorto fulfill your role.

Main duties of the job

You will be responsible for assessing,planning, implementing, and evaluating individualised care for patients underthe direction of a Clinical Team Lead and District Nurse.

You will work in close collaboration withhealth and social care professionals within the Primary Care Network to preventyour patients being admitted to hospital.

You will provide advice and support tojunior staff and students in the delivery of effective, high-quality care.

Job description

Job responsibilities

The post holder isresponsible for working as part of the community nursing team to assess, plan,implement and evaluate holistic care for patients in the community. Observing patients conditions and reportingto the senior nurses and GP members of the team. The post holder will developand maintain knowledge, skill and competencies related to the role of NursingAssociate within the community. The post holder will carry out nursingprocedures such as catheterisation, wound care, medication administration andsupervise and delegate work to students and HCAs.

Person Specification

Qualifications

Essential

Registered Nursing Associate on the NMC register. Nursing Associate foundation degree qualification. GCSE Grade A-C in Maths and English or Functional skills level 2 Maths and English

Experience

Essential

Experience of working in teams under appropriate supervision as part of a multi-disciplinary team. Insight into how to evaluate own strengths and development needs, seeking advice where appropriate. Experience of providing and receiving complex, sensitive information.

Desirable

Previous experience of working within a health/ care setting

Skills

Essential

Ability to take part in reflective practice and clinical supervision activities. Ability to organise and prioritise own delegated workload. Ability to communicate effectively (written, verbal, and nonverbal communication) with members of the public (patients, relatives/ carers) and all members of the multi -disciplinary team. Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact. Ability to develop effective and appropriate relationships with people, their families, carers, and colleagues. Ability to support, supervise, assess, and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting. Completion of an HCA development programme

Desirable

Completion of an HCA development programme Evidence of involvement in support / development of less experienced staff Evidence of recent work-based learning or self-directed learning

Knowledge

Essential

Understanding of the scope of the role of the Nursing Associate in context of the team and organisation, and how the role may contribute to service development. Understands and acts in line with NMC professional standards for practice contained within The Code. Knowledge of when to seek advice and escalate to the appropriate care professional for expert help and advice. Understanding of the importance of following procedures and treatment plans. Understands the requirements for NMC professional revalidation. Understanding of evidence-based practice. Understanding of basic physiology.

Essential
