This is a full time, permanent role and offers a salary of £23,400.nAre you ready to make a real impact in the lives of those transitioning out of homelessness? Do you have a passion for building brighter futures and empowering individuals to thrive independently?nAs a Case Work Coordinator, you will play a pivotal role in managing referrals, assessing client needs, and providing comprehensive support to maintain accommodation and facilitate positive life changes.


nKey Responsibilities:


nBuild positive relationships with external agencies to enhance service delivery.nProgress referrals and track outcomes for service users.nConduct needs and risk assessments, developing tailored support plans.nProvide advice and facilitate access to appropriate services for clients.nSupport clients in accessing meaningful occupation opportunities.nMaintain accurate client records and contribute to data analysis for service improvement.nEnsure effective move-on pathways and manage rent arrears.nProactively manage neighbourhood and community relations.nUphold health and safety standards within the service.nCollaborate with external partners to safeguard clients.nActively contribute to organisational meetings and development initiatives.nParticipate in the on-call rota system.


nSkills/Requirements:


nPrevious work with socially excluded individuals, team collaboration in social or health care settings, and familiarity with complex needs and challenging behaviours.nUnderstanding of homelessness causes and social exclusion, excellent communication skills, and proficiency in Microsoft Office.nHave a full driving license and access to a car for work purposesnThis role will be subject to DBS check at the relevant level.
