Job Summary

Opportunity to join the Skegness Older Adult CMHT, we are looking for a Band 5 Community Psychiatric Nurse/RGN/AHP with relevant experience in mental health or a non-mental health qualification if you can demonstrate transferable skills. Our service hours are Monday to Friday 9 am - 5 pm, providing assessment, diagnosis, and treatment for patients of all ages with memory challenges under the MAMS and dementia pathways, and for older adults facing severe and enduring mental illness.

The successful candidate will offer top-quality person-centered care to patients, families, and caregivers. Collaborate with the MDT within LPFT and the broader network, including Neighbourhood Teams, Adult Social Care, GP practices, and other partner organizations to ensure the best outcomes for patients and caregivers.

The team is situated at Holly Lodge in Skegness, serving 5 local GP practices, rendering care at the workplace, patient residences, and community care setups.

Main Duties of the Job

  • Provide high-standard clinical care for safe and effective assessment, treatment, and comprehensive discharge planning using advanced clinical reasoning skills and appropriate assessment tools.
  • Delegate parts of the patient treatment plan to other team members while maintaining overall responsibility for patient care, when suitable.
  • Maintain the highest standard of clinical care in the clinical caseload and clinical practice of nursing team members in the Community Mental Health Team.
  • Manage caseload and service delivery to contribute to achieving Service/Trust productivity needs.
  • Act as a care coordinator under the Care Programme Approach, ensuring up-to-date care plans, risk assessments, and reviews are in place.
  • Collaborate with individuals to assess their mental health, identify mental illness, and understand and manage their needs and circumstances to enable appropriate behavior modifications.
  • Plan, execute, review, and enhance interventions to address people's identified needs and manage their inherent risks.
  • Evaluate the needs of carers and families, and create, implement, and review support programs for them.
  • Safeguard individuals from abuse, neglect, and harm.

Qualifications

Essential

  • RMN or RNLD or RN: 1st Level Registration (NMC) or degree equivalence.
  • Currently Registered Nurse
  • Evidence of specialized ongoing professional training (degree level) in clinical practice.
  • Mentorship Module (degree level)
  • ENB998 / FliPS or equivalent

  • Management training

  • Relevant experience in working with adults with mental health needs or significant experience in crisis intervention with transferable skills to community/inpatient care.
