You will be working alongside the multidisciplinary team who support children & young people from all over East & North Hertfordshire and the surrounding areas, in the home, schools and in the acute hospital setting.
Communicate sensitive information to patient's families and know when to involve the CYPD MDT team members as appropriate
Maintain medical records systems by filing and keeping an accurate record of the movement of medical records and the case note tracking system on IFIT.
Tracking notes to relevant areas /s on the Lorenzo system and putting patient data, clinics and outcomes on the system.
