We are looking for an Advanced Practitioner to work clinically in the South Molton and Chulmleigh team and to provide expert clinical support and guidance within the team.

The purpose of the post is to provide highly specialist assessment, diagnosis, treatment and advice to patients and their carers. The advanced practitioner leads the clinical specialty providing leadership, development and governance, supporting other clinicians regarding patient treatment and management. You will be part of an interdependent, multidisciplinary team ensuring that therapy input is integrated into the patients’ overall care plan. You will work with the manager to lead and develop the service in line with patient need and trust wide developments and to help provide an equitable service across the Trust.

The community therapy team work within the integrated health and social care team (H&SCT) supporting patients registered to South Molton and Wallingbrook GP practices. The community therapy team includes occupational therapists, physiotherapists, an occupational therapy apprentice, assistant and trainee assistant practitioner and clinical support workers. The integrated health and social care team includes the community therapy team, community nursing, community matron, rehabilitation nurse, adult social care team and coordinator. 

Community Rehabilitation is varied and highly rewarding. We work in a beautiful area of North Devon close to its moors and beaches.

Working Pattern: Monday to Friday, 8am - 4pm.

Interview Date: Monday 4th November 4

Pay:  The salary advertised for this position reflects the Agenda for Change 4-5 rates, inclusive of a pay award implemented from October 4. If you join before this date, the pay award will be backdated to April 4 or your start date, whichever is later. Until then, remuneration will be based on the 3-4 pay bandings (Pay scales for 3/ | NHS Employers).
