Job overview

This is an exciting opportunity for an experienced Care Co-ordinator to join our specialist service for people aged 14-35 experiencing a first episode of psychosis. The successful candidate will work collaboratively with service user, loved ones and will work within the multi disciplinary team to ensure effective care coordination.

Main duties of the job

To ensure effective care coordination and delivery of a nice concordant care package within an assertive outreach model.

Psychosocial interventions for psychosis Training in Behavioural Family Therapy

Experience

Evidence of relevant post registration clinical experience

Desirable criteria

Ability to conduct first line assessments for individuals newly referred into the service, utilizing a biopsychosocial framework.
