As adepartment our key focus areas for development are aimed at our medicinesoptimisation and sustainability agenda. Thepostholder will play a pivotal role in supporting us to achieve thesepriorities through completing drug histories, consulting with service users andcarers regarding medicines, leave and discharge planning, utilising nearpatient dispensing and liaising with primary care to enable smooth transitionpost discharge.

Main duties of the job

The successful candidate willwork with the speciality pharmacists and all members of the MDT to co-ordinateall aspects of the patients medicines related journey, communicatingeffectively to ensure a safe, seamless and integrated transfer of care for ourpatients.

Key result areas include:

Supporting clinical teams with the supply,storage and management of medicines within inpatient areas.

Complete medicines history/reconciliation whenpatients are admitted into our care.

Provide information to patients and carersabout their medicines and support adherence and understanding.

Work closely with clinical teams to plan andsupport timely leave and discharge, utilising near patient dispensing processto ensure medicines are received in a timely manner.

Support with transfer of care into primarycare, utilising the DMS service and liaising with other healthcareprofessionals.

Participate in the accuracy checking rotawithin the dispensary.

Support the training and development oftrainees.

Participate in theweekend and bank holiday rota

Job responsibilities

Band 4

Thisis a post designed to meet the requirements of the pharmacy on the Fieldheadsite.

Todispense medicines and provide technical support to the mental health pharmacyservices including checking patients own medication on admission, thedispensing and supply of medication to specific patients and units includingin-patient wards, forensic, learning disability and crisis services.

To ensure patients information leaflets areavailable on allocated units and in identified areas and keep all pharmacyinformation up to date

Toensure stocks of medicines are available on all allocated wards and departmentsthrough the topping up service.

Toassist in in the bank holiday provision of pharmacy services at Fieldhead andto be involved in the Saturday morning rota.

Towork in other areas of the Trust (by previous agreement) if necessary.

Band 5

Thisjob description is an outline of the main duties of the post. The postholder will be required to undertakeother duties commensurate with the grade as directed.

To provide award pharmacy service to allocated units external to hospital sites

To providetechnical support to the mental health pharmacy services including checkingpatients own medication on admission, the dispensing and supply of medicationto specific patients

Co-ordinateand manage the day-to-day workload in the dispensary. Provide immediate linemanagement support for allocated staff (pharmacy technicians band 4, pharmacyassistants or student technicians)

Provide day today line management for the dispensary staff

To accuracycheck completed prescriptions and maintain technical competency for this)

Person Specification

Training

Essential

Band 4: NVQ Level 3, Btec or similar nationally recognised qualification in pharmaceutical sciences. Up to date continued professional development (CPD). Band 5: Nationally recognised accuracy checking qualification or has ability to demonstrate competencies at this level. Accredited Checking technician status.

Desirable

Band 4: Health and Safety. Medicines Management. Mental Health. Band 5: Medicines management training. Team leading training.

Qualifications

Essential

Band 4: NVQ Level 3, BTec, or similar nationally recognized qualification in pharmaceutical sciences. Registered with the General Pharmaceutical Council (GPHC). Band 5: NVQ Level 3, BTec, or similar nationally recognized qualification in pharmaceutical sciences UK Professional registration as a Pharmacy Technician

Desirable

Band 4: Medicines Management Training. Band 5: NVQ A1 assessor qualified.
