The Mid Yorkshire Teaching NHS Trust is pleased to announce a fantastic opportunity to join our Medicines Optimisation team as a Pharmacy Services Assistant (Advanced Level). This role involves collaborating with a clinical Pharmacy team within a ward setting to ensure that patients receive their medications safely and effectively.
The ideal candidate will possess an NVQ Level 2 certificate in Pharmacy Services or an equivalent qualification recognized by the GPHC. Experience in a pharmacy setting, whether in a community or hospital environment, is essential. A passion for patient care, along with strong attention to detail, self-motivation, and excellent organizational and communication skills, are key attributes we seek.

Key Responsibilities
As a vital member of the Pharmacy Medicines Optimisation team, your responsibilities will include:
- Engaging with patients to evaluate the safety and suitability of any personal medications they may have brought, documenting this on the electronic prescribing system.
- Ensuring patients have an adequate supply of medications during their hospital stay to prevent missed doses by utilizing the electronic prescribing system for orders.
- Collaborating with pharmacy and ward staff at all levels to enhance the efficiency of the service.
- Dispensing prescriptions for patients to take home, facilitating timely discharges, and assisting the Pharmacy team in providing medication counseling.
- Transferring medications between wards as necessary.
- Assisting nursing staff with medication inquiries and orders.
- Contributing to the training and development of Pharmacy Support Workers, Trainee Technicians, Pharmacy Technicians, and Trainee and Foundation Pharmacists.
The Pharmacy Department operates a 7-day service throughout the year, and participation in weekend and Bank Holiday rotas is expected. The role may also involve cross-site responsibilities and late shifts.

Job Duties
- Print ward lists and handover sheets to identify newly admitted patients since the last pharmacy visit.
- Request and document patient consent for referrals to discharge medicine services and community-based pharmacy services.
- Transfer medications between wards for patients who have been relocated without their medications.
- Support ward pharmacists and technicians in accurately recording medication histories by assessing patient-provided medications.
- Ensure patients have sufficient medications to avoid missed doses by evaluating personal medications for suitability.
- Communicate with relatives or caregivers, with patient consent, to bring in necessary medications if required.
- Review prescription charts to identify missed doses and new prescriptions, requesting supplies from pharmacy as needed.
- Collaborate with ward staff to identify additional stock medications required.
- Counsel patients on new medications, including side effects and interactions, using available systems and training.
- Maintain safe and secure handling of medications by ensuring proper storage and delivery to bedside lockers.
- Participate in pharmacy-led audits as required.
- Report any incidents of improper storage and handling of medications to the appropriate personnel.
- Ensure timely communication of collected information to the relevant pharmacist or technician.
- Minimize medication waste by managing returns from clinical areas according to standard procedures.
- Dispense medications for inpatients, including discharges, under supervision, with final checks by a qualified technician or pharmacist.
- Record necessary information for accurate stock control and prescription data using both paper and computerized systems.
- Liaise with staff regarding urgently needed medications and inform patients and healthcare staff about supply issues.
- Dispense specialist and unlicensed medications, ensuring accurate record-keeping.
- Dispense oral chemotherapy medications, adhering to necessary precautions.
- Dispense controlled substances, ensuring all legal records are accurate and up to date.

Qualifications
Essential
- NVQ Level 2 in Pharmacy Services or equivalent (mandatory training for all Pharmacy support staff by the Pharmaceutical Society).
- Reasonable educational background, particularly in Mathematics, English, and Science.
Experience
Essential
- Experience in a pharmacy dispensary (NHS or retail sector) and in a customer service environment.
Desirable
- Previous experience within the NHS.
