Job summary

An exciting opportunity has arisen for an experienced, enthusiastic, and well-motivated specialist rehabilitation and respiratory Physiotherapist to provide input to the Critical Care unit at the new Royal Liverpool University Hospital (LUHFT). You will be part of an innovative team currently providing inpatient services across several specialist areas and will work alongside the current Critical Care Clinical Specialist and Critical Care physios and MDT to provide ongoing input.

The post holder will sit within one of the largest in-patient therapy teams (critical care, medicine, and surgery) at LUHFT. This team comprises of multi-professional team members working within an interdisciplinary model covering critical care, HPB/surgical wards, medical wards and non-invasive ventilation.

The successful candidate will be directly responsible for the provision of physiotherapy to intensive care patients with a significant emphasis on complex respiratory management, rehabilitation and ventilation/tracheostomy weaning. As a progressive team we are equipped to progress and support senior staff members on their chosen career path in ICU.

The post holder will be on-call competent and comfortable working in a challenging, acute, fast paced environment and able to offer support to staff in the management of clinically challenging patients. There is an expectation that the post holder will support the weekend and late shift services at LUHFT.

Main duties of the job

To provide clinical assessment/evidence-based treatment for complex critically ill and high dependency patients, including tracheostomy management and ventilatory weaning plans. To design and deliver training and development of junior staff members and MDT to develop clinical skills, including on call competency. To support a busy team of Physios, OTs, TAs and APs alongside the existing Band 8a team leader and Band 7 staff, providing clinical and pastoral supervision. To support with data collection and performance monitoring of the clinical care provided on critical care and use this data to identify areas of the service needing development. To support with rota development and monitoring. To support the Band 8a clinical specialist and therapy lead in performance monitoring and management of the team including regular data collection and information gathering to contribute to monthly care group reports. To provide supervision and appraisal to bands 6-3 To co-ordinate care for critical care patients with ongoing rehabilitation needs with the Cheshire and Merseyside Network. To be involved and support special interest and local peer support groups

CLINICAL RESPONSIBILITIES

To have overall accountability for the planning and management of a highly complex and specialist clinical caseload

Using expert specialist knowledge to undertake complex therapy assessments including evaluation of the impact of patients' diagnoses, stage of disease and agreed treatment plans on their ability to function and overall quality of life and make clinical decisions on the most appropriate form of intervention, both short and long term

To reflect on clinical practice both individually and with peers identifying strengths and needs and providing evidence of sound clinical judgements based on research and evidence-based practice

To facilitate the production of clear and evidence-based resources for patients and carers , for use locally, regionally or nationally

To be competent in the use of relevant equipment. This may include training of other healthcare professionals in its correct use

To provide expert advice to all members of the multidisciplinary team in a range of forums rounds and multidisciplinary meetings and case conferences

To participate in on call duties as an autonomous practitioner within the Trust

and actively participate in the weekend service

2. RESPONSIBILITY FOR PATIENTS

To facilitate patient involvement in decision making ensuring that they have consented to therapy intervention in line with Trust policies and the relevant Code of Professional Conduct

To provide expert therapeutic advice to patients, carers and clinicians using evidence-based practice or advanced clinical judgment acquired through professional expertise

To use highly advanced communication, motivational interviewing and behaviour change skills to negotiate therapy interventions in situations where sensitive information needs to be discussed

To translate highly complex scientific, medical, biochemical and therapeutic information into simple and easily understood practical information and advice for patients, their carers and families

To monitor and review the progress of clients/ patients against goals and outcomes set and to modify these as appropriate

3. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT

To contribute to service planning in liaison with other team leads, clinical lead and/or the therapy service inpatient manager.

To contribute to the production of relevant evidence-based departmental literature in the specialist clinical area

To participate in uni-professional or multidisciplinary working groups, questioning and evaluating current clinical practice to provide consistently high standards of patient care

To support in developing patient-focussed, clinically effective and evidence-based standards, procedures and policies

To participate in local and national initiatives relating to the specialist clinical area, National Service Frameworks and NICE guidance

To work alongside the clinical lead and in their absence be responsible for representing the profession in developing Trust clinical policies and services within the specialist clinical area

To demonstrate an understanding and assist in implementing appropriate Clinical Governance and Risk Management strategies

To undertake a training needs analysis for the team , ensuring that training needs are identified and action plans initiated

To participate in the development of competencies and standard work objectives

To ensure safe practice through documentation and reporting of critical incidents as outlined in the Trust policy

To actively participate in local professional specialist groups to promote best practice across the profession

Ensure Therapy staff undertake continuing professional development appropriate to their clinical role and personal development in order to maintain professional competence including mandatory training and ensure continued registration with the Health Professions Council

To undertake mentoring and clinical supervision of members of staff
