Job summary

As a paramedic in a primary care setting, you will work aspart of the new Collaborative Home Visiting Service (CHVS) team for Great Yarmouthand Northern Villages PCN to provide holistic care for individuals. You willassess, diagnose and manage the care of patients. This might include referringand signposting them to other allied health professionals.

Main duties of the job

The role is for a paramedic to support primarycare as part of a Collaborative Home Visiting Service (CHVS) team that operates during the working day, Monday toFriday. The paramedic will work as part of the CHVS teambut with close collaborative working with other members of theMultidisciplinary Team and support from GPs.

To work within the community, as an autonomous,accountable paramedic, visiting patients in their place of residence.

To provide holistic care for individualsincluding assessment, diagnosis, and management. To refer and/or signpost patients in their ownhome to other allied health professionals. The post holder will use advanced clinical andcommunication skills to provide education to patients, promote self-care andempower them to make informed choices about their treatment. Care occasionally may need to be provided inother settings as required.

The Great Yarmouth and Northern Villages Primary CareNetwork is a collaboration between the GP practices of East Norfolk MedicalPractice, Coastal Villages, The Park Surgery and Fleggburgh Surgery. Thepractices are working together to deliver high quality, patient-centredintegrated care service to improve outcomes for our local population, bringingtogether local health and care organisations (including social care, thevoluntary sector and patient groups) to work collaboratively as part of the NHSLong Term Plan.

Clinical duties

Assessing and treating patients presenting withundifferentiated and undiagnosed problems in their own place of residence. Instigate necessary invasive and non-invasivediagnostic tests or investigations and interpret findings/reports. Assess, triage and treat patients, making anynecessary referrals to other members of the primary healthcare team or organiseadmission if required. Have knowledge of Long-Term Conditionmanagement. Ensure clinical practice is safe and effectiveand remains within boundaries of competence, and to acknowledge limitations. Advise patients on maintaining health and referto allied services when a health promotion need is identified. Support anticipatory care plans when required. Communicate at all levels within themultidisciplinary team ensuring an effective service is delivered. Maintain accurate, contemporaneous healthcarerecords appropriate to the consultation. Ensure evidenced-based care is delivered at thehighest standards ensuring delivery of high-quality patient care. Work within local policies and procedures. Where the post holder is an independentprescriber: to prescribe safe, effective and appropriate medication as definedby current legislative framework. Enhance own performance through ContinuousProfessional Development, imparting own knowledge and behaviours to meet theneeds of the service. Achieve and demonstrate agreed standards ofpersonal and professional development to meet the needs of the service. Participate in the audit process, evaluation andimplementing plans and practice change to meet patient need. Actively participate in mentoring andsupervision, plus practice derived CPD opportunities. Contribute positively to the effectiveness andefficiency of the team and work colleagues. Take responsibility for safeguarding childrenand vulnerable adults and work with other team members to ensure they areprotected.

Training and personal development

Training requirements will be monitored byyearly appraisal and will be in accordance with PCN and HCPC requirements. Personal development will be encouraged andsupported by the PCN. It is the individuals responsibility to remain up todate with recent developments.

Liaison

Work closely with the multi-professional,management and administration teams to ensure the smooth running of the CHVSteam, reporting any problems encountered to the relevant person and ensuringeveryone is aware of the different roles within the team. Work closely with the wider PCN and Community team. Engage with the Secondary Care Discharge team tosupport the safe discharge of patients back to their home environments. Participate in the education and training ofstaff and students of all disciplines. Maintain continued education by attendance atcourses and study days as deemed useful or necessary for professionaldevelopment, ensuring CPD requirements are met. If it is necessary to expand the role to includeadditional responsibilities, full training will be given. Develop and maintain a Personal Learning Planand educational portfolio.

Meetings

It will be necessary to attend and contribute tovarious PCN and MDT meetings as requested. The only reason for not attendingwill be annual, study or sick leave or if a non-working day.

While seeking treatment, patients entrust uswith, or allow us to gather, sensitive information in relation to their healthand other matters. They do so in confidence and have the right to expect thatstaff will respect their privacy and act appropriately. In the performance of the duties outlined inthis job description, the post-holder may have access to confidentialinformation relating to patients and their carers, practice staff and otherhealthcare workers. They may also have access to information relating to thepractice as a business organisation. All such information from any source is tobe regarded as strictly confidential. Information relating to patients, carers,colleagues, other healthcare workers or the business of the practice may onlybe divulged to authorised persons in accordance with the practice policies andprocedures relating to confidentiality and the protection of personal andsensitive data.

Qualifications, experience and attributes

Essential

Registered paramedic with 5 years post registration experience. Excellent communication skills. Demonstrable patient assessment skills. Knowledge of general practice related medical conditions, treatments and pharmacology. Knowledge of maintaining confidentiality and implementing data protection legislation requirements. Knowledge of safeguarding principles. Ability to work as a team member and autonomously within own scope of practice, understanding when to refer to GPs. UK Driving Licence and own vehicle.

Desirable

Non-medical prescriber qualification. Ability to use SystmOne clinical system.
