Job summary

We have an exciting opportunity for a Rehabilitation Instructor to be part of our Community Respiratory Team which comprises of Respiratory Nurse Specialists and a Specialist Respiratory Physiotherapist,

The role is to assist and support the community respiratory team to implement service provision for patients to access home and clinic based pulmonary rehab To undertake highly skilled support work to facilitate the rehabilitation and education of patients, their families, carers and other professionals involved in their care.

The individual will also be required to support the community respiratory nurses in the management of the patient caseload in clinic and in the patients usual place of residence.

Main duties of the job

To support the provision of pulmonary rehabilitation have knowledge of exercise prescription and advising, educating and using various strategies to modify and improve patient performance during treatment sessions both in a group setting and on an individual basis.

To hold responsibility for own caseload, working without direct supervision, and in doing so being able to perform re-assessments of patients by questioning, observing and measuring patient's abilities and needs. Support will be available from HPC registered physiotherapists and Specialist respiratory Nurses via a variety of communication methods although this may not be in the direct physical vicinity.

To develop and deliver individualised treatment programmes of rehabilitation, in patients usual place of residence.

To be responsible for the safe & competent use of gym equipment, patient appliances and aids used by patients.

To modify and progress exercises of patients either individually or within groups according to care plans set by registered physiotherapists.

To link with exercise referral schemes and other agencies promoting health and wellbeing to promote improved integrated care with community based provision appropriate to the needs of the population, taking into account local deprivation and travel plan issues.

To deliver educational sessions to patient groups to promote self-management of their condition.

Main Duties and Responsibilities

To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence.

Support takes the form of regular formal and informal training and feedback sessions. Access to advice and support from HPC registered physiotherapists is available on a regular basis.

To be responsible for the safe & competent use of gym equipment, patient appliances and aids used by patients. Ensure that Physiotherapy students attain competency prior to use.

To modify and progress exercises of patients either individually or within groups according to care plans set by registered physiotherapists.

To link with exercise referral schemes and other agencies promoting health and wellbeing to promote improved integrated care with community based provision appropriate to the needs of the population, taking into account local deprivation and travel plan issues.

To provide expert knowledge and information on exercise principles to other healthcare staff including training programmes as required.

To deliver educational sessions to patient groups to promote self-management of their condition.

To maintain a clean, tidy and safe environment and equipment.

To participate in audit and outcome measure data collection and reporting as required; carry out clerical/administrative duties as required.

Clinical Duties

To gain informed consent for treatment in accordance with Trust policy. This will frequently be to patients/carers and relatives who have diverse or complex emotional, physical and psychological conditions deafness, dysphasia, depression, chronic pain, etc. and will require expert use of verbal and non-verbal communication tools.

To undertake clinical re-assessments of the patient gaining relevant information by clinical examination, observation, questioning and discussion to independently evaluate patient progression towards their goals and to inform progression/ modification of the patients rehabilitation programme including seeking advice from the referrer if the patients condition is deteriorating or not improving as expected.

Facilitate, monitor and evaluate goal setting with patients to optimise their potential and promote self-management, taking into account the promotion of their physical and emotional wellbeing and disease prevention.

Facilitate self-management strategies and be involved in formulating long term condition management plans to promote ongoing self-management and a smooth transition to third Develop and deliver an exercise programme based on patient education, individualised exercise programmes or classes.

To be responsible for the safe and competent use of gym equipment and patient appliances by self and patients. eg mobility aids.

To maintain clinical records/reports and follow guidelines, protocols and procedures in accordance with professional (Chartered Society of Physiotherapy), legal and departmental standards.

To support the Respiratory Specialist Nurses in the management of their caseloads once competencies have been achieved observations, inhaler technique.

Communication

To communicate effectively with patients, carers and relatives on an individual or group basis to maximise rehabilitation potential through verbal and non-verbal methods. This may require explanation, instruction, persuasion, motivation, negotiation and reassurance.

To be committed to a team approach in order to provide continuity and efficiency of service provision.

To liaise with multi-professional team/colleagues/medical staff as required.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes

The post holder must adhere to infection control policies and procedures

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and other audits as required

Provide patients and their families /carers with information on standards they should expect from the team

Participate in clinical supervision on a regular basis

The post holder is required to participate in relevant emergency preparedness process for their team.

Person Specification

Qualifications and Training

Essential

Exercise Instructor qualification - premier training or YMCA level 3 Relevant professional qualification NVQ/REPs Level 3 with evidence of an anatomy/physiology base Evidence of training in rehabilitation/exercise prescription

Desirable

A sports/physiology related degree Evidence of managing patients in group based exercise classes Evidence of CPD portfolio GP exercise referral qualification Knowledge of Strength and Conditioning and/or working towards an UK SCA qualification

Experience

Essential

Previous experience of rehabilitation or gym instructor, delivering exercise based programs on an individual and/or class based level Experience of working in community environment Experience of working as part of a team

Desirable

Previous experience of working within a healthcare setting (ideally NHS) In stock taking and ordering Experience and evidence of working on service improvement projects
