Job overview

We are embarking on an ambitious and exiting journey to improve our KCHFT Community Hospital pathways, and are looking for an Operational Manager to join our leadership team.

We have a portfolio of 9 Community Hospitals across Kent, and a team of nurses and therapists delivering care and rehabilitation to frailty patients. Our hospitals deliver specialist pathways in Stroke and Fractured Neck of Femur, and support end of life and dementia patient care.

Main duties of the job

As the Operational Lead you will hold overall responsibility and accountability for operational management of Community Hospitals. 

You will manage and provide clear leadership for the staff, patient caseload and resources of the services reporting to the Head of Service and Assistant Director as required. 

A focus will be on standardising our approach through pathway and policy development; which will be guided by national and local guidelines using evidence-based practice to achieve the best possible outcomes for patients.

Whilst working closely with all providers of care across organisational boundaries including primary, secondary, community and voluntary care organisations to manage the patient throughout their care pathway

Additional Working Pattern: Mon – Fri 08:00 – 16:00. On call rota requirement

Person specification

Qualifications

Essential criteria

Degree in Nursing or Nursing related subject or for AHP first contact practitioner qualification at Degree level. Current state registration Management experience, leadership qualification or equivalent. Post registration qualification at degree/ masters level.

Desirable criteria

Experience / qualification Long-Term Conditions, Transformation and Quality Improvement experience. MSc. Management qualification

Experience

Essential criteria

5 years minimum post registration experience and 3 years minimum at senior clinical level within relevant setting. Experience in medicines management. Experience in LTS/Complex care management. Experience of managing and setting clinical treatment plans for patients with complex multi-pathology. Evidence of enhanced clinical skills. Demonstrable experience of working across traditional boundaries. Experience of managing and reporting Adult Protection alerts.

Knowledge

Essential criteria

Clear understanding of the modernisation agenda across health and social care to influence service development and achievement of targets. A clear understanding of clinical governance issues and controls assurance standards.

Skills & Abilities

Essential criteria

Excellent communication, negotiation and influencing skills. Strong evidence of effective leadership and management skills. Skilled in time management, prioritising workload and self-management. Ability to travel to all sites within the Trust area. Ability to make decisions and deal with conflict. Resilient, flexible, self-motivated and open to change.
