Job overview

The post holder will provide managerial leadership for Continuing Health Care (CHC) teams across Tower Hamlets and Newham. The post holder will be the catalyst bringing together two the teams in one space, ensuring that each team meets their individual contractual obligations. They will be accountable for ensuring teams have appropriate staffing levels including the skill mix meets the demands of Tower Hamlets and Newham.

They will work in an open and transparent way, ensuring that succession planning is embedded at every level. 

The post holder will be an expert CHC clinician with extensive CHC knowledge demonstrated by extensive CHC experience. They will demonstrate expert knowledge of CHC policy at a local and national level providing assurance to the lead nurse that teams are competent and proficient in delivering this highly specialised care.

Main duties of the job

·Act as a subject matter expert ensuring that national & local KPI’s are met

·Ensure the team met Trust / regulatory body requirements

·Undertake regular training needs analysis reviews, providing / commissioning training to meet demand across services.

·Ensure the CHC Framework is applied to the highest standard across fully funded / joint funded patients

·Act as the Adult Safeguarding lead in the service working collaboratively with the lead nurse / local authorities as required.

·Make regular service reviews, ensuring the patient remains at the center of care.

·Lead the introduction evidence-based packages of care keeping pace with national / local guidelines developing interdisciplinary care pathways, protocols and audits

·Provide expert clinical advice to service users, carers and colleagues. 

·Undertake quality assurance at an expert level for activities including, research and patient / friends & family survey

Detailed job description and main responsibilities

To provide expert clinical guidance to the Adult Continuing Healthcare Panel.

Lead case management ensuring evidence based care in embedded

Lead KPI compliant CHC assessments

To ensure that nursing and Continuing Healthcare assessments are completed to highest standard implementing action plans to improve practice / service progression as required. 

Represent CHC teams at local / national Continuing Healthcare Panels / leads meetings.

To work closely with a range of healthcare providers and independent nursing care homes, reviewing individual cases for eligibility.

To lead the CHC team in the review of CHC cases for eligibility of funding where required.

Education/ Qualification/ Training

Essential criteria

Registered nurse on Part 1 of the NMC register Relevant post graduate qualifications at Masters level or working towards this Relevant post graduate mentoring qualifications Non-medical prescribing Expert specialist knowledge in continuing health care including evidence of continuing professional development with particular relevance to Continuing Healthcare

Knowledge, Skills and Experience

Essential criteria

Significant post registration experience managing large teams Current, active clinical practitioner at a senior level within NHS Continuing Healthcare demonstrating expert knowledge of local & national policy / guidelines. Experience of partnership working and managing staff across organisations Effective specialist budget management Responsible for ensuring the safe use of specialist equipment related to continuing care service. Experience of achieving change management Experience of facilitating groups Experience of planning and organising of a broad range of complex activities e.g. while providing specialist continuing care nursing service for patients and while delivering training for staff and service users. Working with and within a diverse community.
