We are looking to recruit two Social Workers, who will specialise in assessing/supporting users who may be eligible for Continuing health care funding.

In this role you will work closely within operational social work teams, as well as the internal CHC team and the local ICB; experience of assessing under the Care Act and working in an Multi Disciplinary Team setting is essential.

Specific requirements for this role include:

• Having a good working understanding of the CHC national guidance and framework
• Ability to gather, analyse, and present health and social care needs
• Acting as a source of information and providing advice to colleagues on CHC procedures
• Contributing to the development of CHC knowledge across the department
• Ability to work in a person-centred and strengths-based way to residents
