Job summary

This is an exciting opportunity to join the CNWL NHS sexual health services in the London Borough of Barnet as a specialist sexual health nurse supporting people with learning disabilities (PWLD) into sexual health as part of "The Bridge Service". The Bridge Service is a dedicated clinic for PWLD in Edgware and Vale Drive CNWL sexual health clinics.

The role involves:Improving access and bridging any gaps for PWLD to sexual health advice,information and the full range of clinical services.Providing free, confidential and supported access to the Barnet sexual healthclinics for PWLD as well as working in the community, supporting family carers and support workers and other involved professionals.Providing reasonable adjustments to support patients such as longerconsultations, easy-read and accessible information.Supporting health, social and VCS networks to identify PWLD and improveengagement and planning care.Community outreach sessions providing relationships and sex education,asymptomatic STI screening in community learning disability settings,including colleges and day services.Providing consultations to assess sexual health needs, consent, safeguardingand concerns around capacity; make reasonable service adjustments and referrals as appropriate.Provision of LARC and integrated sessions within the main Barnet sexualhealth full driving licence with access to a car is essential

Main duties of the job

Health Promotion and Disease Prevention (Public Health).

Health Promotion and Disease Prevention / Public Health when in Clinical Practice.

Proactively develops, implements and monitors public health and health promotion activities, including own clinical work, work of team and the local community in accordance with local and national plans.

To participate in clinical and educational outreach and participate in health promotion activity, contributing to the development of new services.

Participates in and influences local health policy development, including health impact assessments.

Engages and forms links with relevant stakeholders and professional agencies, providing expert training and advice where appropriate.

Identifies the needs of diverse/vulnerable groups, young people and contribute to the development of tailored services to meet their needs.Undertakes relevant need assessment profiles.

Promotes safer sex and provides sexual health information to clients as up to date with and prevents cross infection through application of appropriate techniques.

Management of / provision of care for quality standardsWorks unsupervised in the assessment, implementation and evaluation of client care

Improving access and bridging any gap for PWLD and carers to sexual

other commissioned sexual health services.

Providing free condoms and lube and access to the C-Card for young people. Supporting health, social and VCS networks to identify PWLD, make reasonable service adjustments, assess sexual health needs and refer to appropriate support services. Establishing seamless pathways between sexual health services and statutory, VCS and primary care services. Improving engagement of PWLD and carers in planning care. Embedding health promotion advice and support acrosspathways so we make-every-contact-count towards reducing health inequalities. Person Specification

Education/Qualifications

Essential

oRGN (NMC Sub-part 1 Adult branch) oUniversity certificate or equivalent in Sexual Health training oDFRSH/University Certificate in Contraception/ Reproductive Health or equivalent recognized training oIndependent Nurse Prescriber or willing to train oEvidence of continuing professional and personal development

Desirable

oLOC SDI or equivalent qualification oLOC IUT or equivalent qualification oCertificate in mentorship course

Skills / Abilities
