Job summary
We are looking for a GP to join our team, for up to 6 sessions per week, who will make him/herself available to undertake a variety of dutiesincluding surgery consultations, telephone consultations and queries, triagecalls, triage visits, checking and signing of repeat prescriptions and dealingwith queries, paperwork and correspondence in a timely are seeking a caring and committed Doctor who is able to demonstrate high standards of clinical care, and is willing towork as part of a highly motivated team. Informal visits are welcome to answerany other questions and for us to get to know each other before interview.
Main duties of the job
You will be expected to undertake:
Face to face consultations
Telephone consultations
Home visits to palliative and housebound patients and Nursing home residents
Total triage of requests for access
Patient and external provider queries
Checking and signing of repeat prescriptions and issue of acute medication.
Dealing with queries, paperwork and correspondence in a timely fashion.
Work towards meeting targets for Qof, DES, LES and IIF
Our clinical system is EMIS Web. We use eConsult Smart box to triage all requests. Our patient communication is supported by Accurx text messaging.

The Practice has invested in the training of our staff at all levels tomeet the demands of a busy General have a nursing team comprising Nurse Practitioners, Practice Nurses, Assistant Practitioners, Health Care Assistants. We are supported administratively by a team of over pull together as a team, we help each other out when we are busy, andwe believe in a partnership where we all play an equal role and support each will play an influential part in moving the Practice forward, wevalue all our staff. We support each other and want to hear your views, wewont just listen, we will do something about it.

We are active in Warrington ICB; one of our Partners is part of Quality team. We work actively as part of South Warrington Network (SWAN) of which one of our Partners, Dr Ahluwalia is Clinical Director. We are also supported by ARRS roles through SWAN - First Contact Physio, Mental Health Practitioner, Clinical Pharmacist, Paramedics, Social Prescriber and Care Co-ordinator.

Job responsibilities
OverallPurpose to provide NHS personal medical services to registered patients onthe Practice list.
Job Summary:
Thepost-holder will manage a caseload and deal with a wide range of health needsin a primary care setting, ensuring the highest standards of care for allregistered and temporary patients.
Clinical Responsibilities:
Inaccordance with the Practice timetable, as agreed, the post-holder will makehim/herself available to undertake a variety of duties including surgeryconsultations, telephone consultations and queries, triage calls, triagevisits, checking and signing of repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.
Respondto medical problems presented by temporary residents as required.
Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganization.
Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems.
Screeningpatients for disease risk factors and early signs of illness.
Inconsultation with patients and in line with current Practice disease managementprotocols, developing care plans for health.** ?
Toassist in the establishment of appropriate systems to manage common chronicmedical conditions.
Completionof clinically related administrative and non clinical duties needed for thedelivery of the service.
Providingcounselling and health education.
Referpatients for further opinions and interventions according to Practiceprotocols
Recordingclear and contemporaneous consultation notes to agreed standards.
Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions wherever possible).
Prescribingin accordance with the Practice prescribing formulary whenever this isclinically appropriate.
Participatein training plans of medical students as required.
Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
Maintaining, monitoring and updatingcomputer database and management of medical records and practice audits
Ensurethat practice clinical database is updated promptly and accurately withinformation relating to patient records, as required for clinical accuracy andfor practice recording requirements under QOF and Enhanced Services.
Appropriateuse of read codes/snomed codes.
Participatingin the collection and collation of statistics towards the preparation ofpractice reports, performance monitoring and audits
Practice Organisation
Processand action incoming patient and hospital correspondence
Attendanceat in-house meetings, significant event audit meetings and others as deemednecessary
Ensurethat the practice complaints system is adhered to at all times
Awarenessand compliance with all relevant guidelines data protection,confidentiality and health and safety
Maintaincurrent CPR and anaphylaxis certification
