Key Responsibilities:

Caseload Management: Take charge of managing a caseload, delivering high-quality mental health care services, and offering valuable support and advice to other healthcare professionals. Service Development: Contribute to the growth and enhancement of our mental health services, ensuring we remain at the forefront of best practices and innovation. Education and Mentorship: Create an enriching learning environment for University of East Anglia (UEA) students, guiding the next generation of mental health professionals. Documentation & Quality Assurance: Be responsible for the precise clinical documents of patients during admission, discharge, and review, ensuring seamless communication and information transfer across Trust teams and services when appropriate and necessary.

Main duties of the job

About the Role: As a Community Mental Health Nurse, you will play a pivotal role in the planning and delivery of direct patient care. You will work closely with a supportive team to ensure continuity of practice, providing essential mental health care services to individuals in need.

The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment.

Managing a caseload and the provision of Mental Health Care services to people with mental health care needs. Support and advising to other health care professionals. To support aspects of service development. Provide an appropriate learning environment for relevant University students. To be responsible for accurate mental health documents of all patients on admission, discharge and at review. Have an excellent knowledge of mental health conditions, relevant medications and treatment. Have experience or an understanding of working with people who have dementia. demonstrate ability to produce excellent highly detailed, assessments and risk management plans

This is an exciting opportunity to relocate and work 9-5 hrs between Monday to Friday.
