Norwich, United Kingdom Norfolk and Suffolk NHS Foundation Trust Full time

We are recruiting a Primary Care Network (PCN) Mental Health Practitioner to join our team. This role is open to a Nurse, Occupational Therapist or Social Worker.

Now is a really exciting time to join us as we launch our Primary Care Network across the whole of Norfolk and Waveney. Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of this integrated health and care system, which brings care closer to home for our population.
