Job Opportunity

We are seeking a dedicated Band 6 Triage Mental Health Practitioner to join our team at The Sophie Centre.

The successful candidate will play a crucial role in ensuring patients receive timely and appropriate care, working collaboratively with other healthcare professionals.

Responsibilities
  • Triage and assess new referrals to the team
  • Coordinate and manage patient transfers
  • Work closely with multidisciplinary teams to ensure seamless care
  • Complete face-to-face assessments in patients' own homes
  • Write comprehensive assessments and risk assessments
Requirements
  • Registered Mental Health Nurse or equivalent professional qualification
  • Experience in mental health triage or similar roles
  • Strong communication and organizational skills
