Our purpose built, modernised premises include an in-house, independent Pharmacy. Our supportive and dedicated team looks after 11,000patients and consists of lead GP, salaried doctors, ANPs, Clinical Pharmacists,MSK practitioner, PNs, HCAs and admin support led by a skilfulmanagement team. Main duties of the job

Dealing with a wide range of health needsin a primary care setting, ensuring the highest standards of care for all.

Duties include patient consultations, home visits, repeat prescriptions andpaperwork/correspondence

Makeprofessional, autonomous decisions

Assessthe health care needs of patients with undifferentiated and undiagnosedproblems

Screenpatients for disease risk factors and early signs of illness

Develop care plans in conjunction with patients

Providecounselling/health education

Prescribein accordance with practice prescribing formulary/generically when clinically appropriate

Maintain an awarenessof and compliance with all relevant practice policies/guidelines

Acommitment to learning and audit to ensure evidence-based best practice, contributing to audit/clinical standard setting within the practice

Attendtraining/events/meetings organised by the practice/other agencies
