At Maytree Court we provide person centred, flexible support to vulnerable people with a range of support needs, this may include supporting adults with a mental health diagnosis, learning disability, autistic spectrum disorder and physical disability. We are looking for a confident, warm, highly motivated and committed individual for the position of Support Coordinator. You will be responsible for supporting the registered manager with the daily running of the service. Your role will include:

Providing respectful personal care, emotional and practical support to service users Assessing service user needs and devising support plans and risk associated documents Collaborating with service users, families, the multi-disciplinary team to maximise outcomes for service users Supervising staff and deploying staff resources effectively to meet the needs of the service Assisting your line manager in ensuring that the service meets all requirements and standards set by commissioners, Creative Support and other stakeholders

Relevant experience and qualifications are required, as are excellent communication skills, a good general standard of education and an innovative approach to providing person centred support that embodies Creative Support’s ethos of choice, opportunity, and wellbeing.
