Job summary

This is an exciting opportunity to join the Stroke Speciality Nursing team as a full time Stroke Clinical Nurse Practitioner, working days, nights, weekends and bank holidays. We are looking to recruit a flexible, enthusiastic and committed professional with a passion for ongoing development for knowledge and skills in stroke care. You will work in a range of care settings throughout Royal Blackburn Hospital including the Emergency Department, Acute Stroke Unit and Ward settings. This fast paced and often challenging role provides an essential resource to ensure patients who require stroke speciality input in the hyperacute phase, are provided with time critical interventions efficiently and safely. Organisation and prioritisation plays a key role in successful management of stroke patients and having the ability to effectively orchestrate multidisciplinary team involvement enables a high standard of speciality care.

You will be expected to demonstrate the ability to work alone as well as part of a team and develop sound clinical assessment skills to reflect a Stroke Speciality nurse standard, enabling you to confidently advise others in the management of stroke care. Speciality training will be provided whilst in post and competency will be supported to achieve the desired skill set to undertake the role independently.

As part of the role, data collection, audits and the use of electronic systems for data input is expected and accurate documentation is essential.

Main duties of the job

Work collaboratively within a large multidisciplinary team to provide excellent care in the clinical area as a competent member of the clinical team. Assessment and evaluation of patients needing stroke intervention at initial stages of admission or onset of stroke symptoms in both the emergency departments and inpatient settings and using own speciality skills and knowledge to plan appropriate care. Support the Acute Stroke Unit and areas providing care for stroke patients using knowledge of current guidance in stroke management.

About us

The acute stroke speciality nursing team is comprised of 4 Stroke Nurse Consultants, a Stroke Specialist Nurse and Band 6 Stroke Clinical Nurse Practitioners who work closely with a Speciality Multidisciplinary team to provide effective stroke management for patients across the stroke pathway. We work hard to meet the clinical guidance surrounding stroke management including meeting Sentinel Stroke National Audit Programme targets in order to provide the best possible care to our patients.

The stroke specialist nurse service is a 24 hour service ensuring the ability to provide Thrombolysis out of normal working hours via Telemedicine Stroke Consultant assessments. A Thrombectomy service is available 7 days per week for our patients at the comprehensive stroke centre at Royal Preston Hospital and we work closely with the stroke team and Neuro Interventionalists on that site to ensure appropriate patients are transferred within a timely manner.

Job description

Job responsibilities

Speciality nursing role in the hyperacute/acute management of stroke patients. To identify stroke and TIA patients following admission (A&E / Wards) and ensure the stroke and TIA pathway is followed, including assessment for Thrombolysis and/or Thrombectomy. The practitioner will deliver excellent patient care on a daily basis. They should be an expert in all areas of stroke nursing care, from a patient's admission to discharge and have awareness of the formal stroke pathway. They will actively support in providing education and awareness around stroke to junior members of staff including student nurses and medical students . They will provide a high standard of clinical leadership to nurses, medics and support staff in the delivery of evidence-based, patient-centred care. They are responsible for ensuring all care is delivered according to Trust policies and procedures and National Clinical Guidance for Stroke. Utilising clinical audit, research, supervision of practice and teaching, they will monitor and improve standards of care by efficient and effective use of resources.
