Acute Care Team Advanced Clinical Practitioner

We are looking to recruit a highly motivated and skilled Advanced Clinical Practitioner with both an interest and experience in deteriorating patients. The role requires strong leadership with an ability to drive innovation and strive for quality care.
The Acute Care Team are a team of ACPs, prescribing and non prescribing practitioners with both nursing and allied health professional backgrounds. We deliver services across all sites and rotate between both Royal Blackburn and Burnley General Hospital. Our team cover services across all areas including the emergency department and outpatient areas. We respond to deteriorating patients, support critical care admissions and discharges, out of hours medical cover, flow throughout the hospital, peripheral site clinical site management and elective admissions.
The main duties of the Advanced Clinical Practitioner within the Acute Care Team are to lead a team of practitioners and ensure high quality care for the delivery of services. You will be required to support innovative practice and lead quality improvement, assess, diagnose, and manage clinical presentations, deliver education and undertake research and audit. The post holder will be supported by ACP colleagues and lead by both a Lead ACP and Consultant Nurse.

- Working as an Advanced Clinical Practitioner making independent clinical judgement/decisions based on patients clinical need
- Undertake independent comprehensive assessment and management of patients to incorporate physiological, psychological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes, including, where appropriate, listing patients for elective procedures and discharging patients from the service when treatment is complete/optimum outcome is achieved
- Demonstrate a high level of competence in clinical, diagnostic and examination skills
- Provide counselling and advocacy for patients with acute and chronic conditions
- Act as a facilitator, resource and model to nursing and medical staff, and professions allied to medicine
- Undertake research and service development
- Regularly re-evaluate the service and develop according to need, within the limitation of resource
- Establish close liaison and communication between all disciplines involved in the care of patients and their families and work in partnership with other practitioners.
- Monitor and evaluate the role of the Advanced Clinical Practitioner in liaison with appropriate managers and medical staff, and demonstrate and respond to changing needs
- Develop and sustain close working relationships, partnerships and networks and draw upon multi-agency/inter-professional resources where appropriate in order to improve health.
- Keep up to date with developments in the role of the Advanced Clinical Practitioner through personal professional development, in service training and education, professional conferences, courses and seminars as appropriate
