The Medicine for Older People Directorate at East Lancashire Hospitals NHS Trust is looking to recruit an experienced and highly motivated healthcare professional as a Trainee Advanced Clinical Practitioner in Frailty.

This is an exciting opportunity for an experienced and proactive healthcare professional to develop their practice and train as an Advanced Clinical Practitioner specialising in the care of frail elderly patients within an expanding and motivated team. You will also be facilitated to further develop your skills in leadership, research/quality improvement and education

The successful applicant will undertake a fully funded MSc in Advanced Clinical Practice at The University of Central Lancashire over the two-year training period, while working as a trainee ACP on our ACP-led Same Day Emergency Care Unit (“Older Persons Rapid Assessment Unit”) OPRA. The trainee will have daily support from a wide and far reaching MDT. With the aim of eventually being able to provide support and guidance to non-specialist/junior clinicians in all aspects of frailty care.

As a trainee ACP, you will be supported to improve your clinical skills and knowledge to an advanced level, both during formal university training days and in your day-to-day practice, with the aim to become a clinical expert in frailty. You will receive a high level of supervision and support from the entire clinical team.

The Medicine for Older People directorate have led significant changes to our frailty services over the past year, resulting in a high performing seven-day frailty service. We hope to recruit an enthusiastic trainee ACP to contribute to our continued improvement and expansion of the service.

We are keen to hear from applicants of any professional background with a keen interest in frailty and elderly care. The successful applicant must have the professional and legal ability to prescribe after completing a non-medical prescribing course, which will be undertaken as part of the MSc. The applicant must also be eligible for apprenticeship funding and meet the entry requirements for the MSc in Advanced Clinical Practice at UCLAN.

**Key responsibilities**:

- Undertake clinical and holistic assessment of frail elderly patients in the Older Persons Rapid Assessment Unit and across other clinical areas as needed, under the supervision of a qualified clinician
- Complete an MSc in Advanced Clinical Practice and additional training as required
- Provide guidance and advice to other members of staff to ensure safe and effective care for frail elderly patients
- Act as a clinical leader within the Trust
- Support the ongoing development of frailty services across ELHT
- Work closely with the wider multi-disciplinary team to ensure a high standard of care
