East Lancashire Hospitals NHS Trust is an integrated provider of acute and community care to a population of over half a million. We are seeking a consultant with drive, commitment and enthusiasm who will bring clinical leadership to our multidisciplinary teams and provide high quality care to our patients.

This is a full-time substantive post and supports our ongoing comprehensive reconfiguration of services for unscheduled care which involves further development of our “front door multidisciplinary frailty service”. The successful applicant will share the responsibility for an MfOP ward. They will also support the frail elderly patients on the Older Person’s Rapid Assessment (OPRA) Unit, Older Person’s Units (OPU) and outpatient clinics. We would welcome subspecialty interests in Parkinson’s disease, Community Care of the Elderly, Acute Frailty, Dementia, Ortho-Geriatrics, Surgical Liaison or Stroke Medicine.

In January 2019 the Trust opened an Older Persons Rapid Assessment Unit our Same Day Emergency Care Unit providing multidisciplinary comprehensive geriatric assessment as part of a developing acute frailty pathway. We are seeking established or aspiring clinical leaders to take this service through the next stages of its evolution.

Our main sites, the Royal Blackburn Hospital and Burnley General Hospital are both Teaching Hospitals. We have strong links with the University of Central Lancashire and the University of Lancaster and there are opportunities for research and teaching posts. We are actively involved in education for both undergraduates and postgraduates and contribute to the Health Education North West training programme.

**Responsibilities**:
To take a shared role in the development of the geriatric medicine service, in conjunction with the management team and consultant colleagues in the department of Medicine for Older People

To develop and support sub-acute rehabilitation for geriatric patients in community hospitals and community settings.

To provide an outpatient service in various locations

To provide continued clinical responsibility for the patients under his/her care in conjunction with colleagues in medicine for older people.

To take part in agreed provision of cover for consultant colleagues during leave.

To develop and promote services that minimise hospital stay and maximise opportunities for recovery and independent living.

To participate in medical audit and clinical governance.

To provide professional supervision, direction and training to junior medical staff.

To participate in postgraduate teaching

To attend meetings of the Directorate of Medicine for Older People, Rehabilitation and Stroke.

To be part of the MDT on Older Person’s Rapid Assessment (OPRA) Unit with 2 Older Person’s Units

This post also carries a commitment to support the management of acute illness in older people with on call commitment for GIM, and care of a minimum of 20 acute older/general medical inpatients.

This is a 10 programmed activity (PA) post, with 7.5 PA set aside for Direct Clinical Care (DCC); and 2.5 PA for Supporting Professional Activities (SPA). SPA is important for consultant job satisfaction and includes continuing professional development (in addition to study leave) mandatory training, job planning and preparation for appraisal. This post will contribute to a 1 in 6 weekend cover.

The posts are whole time appointments, however applicants wishing to work less than full-time will also be welcomed and the job plan modified accordingly.
