4 weeks ago

Blackburn, United Kingdom East Lancashire Hospitals NHS Trust Full time

This role is as a Junior Clinical Fellow in Emergency Medicine in one of the busiest and most varied Emergency Departments in the UK. As such you will work in both the Emergency Department which comprises adult and Paediatric areas and the Urgent Care Centre. As such you will have the opportunity to assess unwell and injured adult and paediatric patients, come up with an initial management plan and arrange onward disposal of the patient. The department has Consultant cover up until the early hours of the morning and Middle Grade cover 24 hours per day, so you are well supported. It is the ideal job for someone who wishes to just have a taster of Emergency Medicine prior to doing something else, or those committed to a career in the specialty. It is also a post suitable for those medical staff who haven’t worked in the UK or NHS before as it is so well supported.

The main duty of the role is the initial assessment of adult and paediatric patients and the implementation of a management plan. There is an expectation that most cases that you assess will be discussed with a Consultant or Middle Grade doctor to confirm your plan. You will work in the Emergency Department assessing the patients in the resuscitation and majors areas of the department, the Paediatric area and the Urgent Treatment Centre where patients with minor injuries and illnesses are assessed so you gain experience in assessing a wide variety of undifferentiated patients. There is also the opportunity to learn new clinical skills such as fascia-iliaca block, suturing, local anaesthetic administration, fracture and dislocation manipulation. There is weekly departmental teaching and you get dedicated self-directed learning time for 8 hours every other week to aid your career development by doing things such as supervised quality improvement projects.

Our vision is to be widely recognised for providing safe, personal and effective care. We currently provide high quality services and treat over 700,000 patients a year from the most serious of emergencies to planned operations and procedures. We employ over 8,000 staff, many of whom are internationally renowned and have won awards for their work and achievements.

The department itself has a large multi-disciplinary team encompassing Consultants, Specialty Doctors, Higher Emergency Medicine Trainees, Junior Clinical Fellows, Foundation doctors, General Practice Trainees, Advanced Care Practitioners, Physicians Associates, Emergency Nurse Practitioners and Physiotherapy practitioners. As well as the nursing team. The department also supports the education of medical students from UCLAN and Lancaster Universities. We are a teaching unit providing teaching for all grades of staff.

You will be assigned an educational supervisor who will meet with you regularly to ensure your educational needs are met whilst you are with us and we encourage Junior Clinical Fellows to undertake the Life Support Courses whilst you are with us. To aid this you will have access to a study leave budget. There is simulation training which is run on the shop floor and that you will be encouraged to attend.

During your clinical time you will;
- The main responsibility will be to provide appropriate care for the various patients who attend the Department. This will vary from providing;
- Immediate resuscitation for seriously injured and seriously ill patients
- Assessment and immediate care for other emergency patients
- Referring patients to the appropriate specialty for admission or referral to out-patient clinics, such as Fracture Clinics, or referral back to the General Practitioner.
- The Clinical Fellow in Emergency Medicine and Urgent Care will work on a shift system which varies according to a 21 week cycle. The contracted hours of duty shall be the standard working week of 40 hours and in addition he/she will be available for an additional 3 full shift hours (which includes 4 hours protected teaching).

Management
- Liaising with GPs regarding their patients who attend the Emergency Department and Urgent Care Centres.
- Clinical Fellows appointed to the Department of Emergency Medicine should familiarise themselves with the Major Incident Plan and will be required to participate in any practice exercises.
- Learning & Development
- Teaching experience is easily available even when working clinically. We host final year medical students from UCLan and Lancaster University, as well as nursing, paramedic, trainee ACPs and a variety of allied health professional students.
- The Department of Emergency Medicine provides 4 hours weekly for protected teaching. All Clinical Fellows are expected to undertake 4 hours of self-directed learning during this time. There are on line modules relevant to Emergency Medicine and joint teaching with the Acute Medical Department takes place once a week. Each Clinical Fellow will get a Consultant as an Educational Supervisor who will help them complete appropriate Work Based Placed Asse
