JobSummary:

TheResettlement Team help to rehouse people coming out of prisons andhospitals.

The roleinvolves completing assessments in line with homelessnesslegislation issuing notification and decision letters completingPersonal Housing Plans and moving vulnerable people into suitableaccommodation.

KeyDuties/Accountabilities:

To provideexcellent customer service to homeless applicants and keypartners.

To providespecialist homelessness casework to single applicants with complexsupport needs leaving institutions ensuring that all aspects ofcasework are carried out in accordance with legislation strategiespolicies and initiatives set out at the national regional and locallevel through effective and regular case work supervision adviceguidance andsupport.

Collaboratewith internal and external partners to improve outcomes for singlehomeless applicants with complex needs and housingneeds.

Contribute toembedding safeguarding best practice across the wider HousingAdvisoryService.

Contribute tothe development of the Homeless Casework Service and wider HousingAdvisoryService.

To ensure ourduties under the Homelessness Reduction Act are met delivering aproactive person centred outcome focused and end to end casemanagementapproach.

To undertakecomprehensive assessments that lead to focused and coproducedPersonal Housing Plans (PHP) for eachapplicant.

To assessapplicants under the prevention and relief duties issue S.184 finaldecision letters ensuring effective communication and management ofexpectations withcustomers.

As appropriateundertake the assessment and completion of highquality riskmanagement plans for applicants utilising information from a rangeof stakeholders and the applicantthemselves.

To minimiserisk by identifying reporting and following up any safeguardingconcerns.

Possess andmaintain an ongoing indepth knowledge of relevant welfare andhousing legislation and the Council s statutory housing dutiespolicies andprocedures.

Provide adviceand guidance to applicants by responding effectively andefficiently to enquiries maintaining the highest standards ofprofessionalism and customercare.

Create andmaintain accurate GDPR compliant client records and notes of allcasework activities and decisions through both manual and ITrecordingsystems.

Coordinate theprovision of services and support for applicants in line with thePHP liaising with other teams and both internal and externalservices.

Attend variousmultiagency meetings as required such as discharge planningmeetings and MAPPA (multiagency public protection arrangement)meetings.

EssentialExperienceRequired:

The idealcandidate will have experience within housing dealing withvulnerable people and working under The Homelessness Reduction Act2017.

Significantexperience of providing specialist homelessness casework to singlehomeless applicants with more complex needs such as applicantsleaving institutions including prisons andhospitals.

Significantexperience of working collaboratively to improve outcomes forsingle homeless applicants with more complex needs such asapplicants leavinginstitutions.

Ability andexperience to consistently deliver high levels of customer serviceto single homeless applicants with more complexneeds.

Ability andexperience of ensuring all aspects of casework is carried out inaccordance with legislation strategies policies and initiatives setout at the national regional and locallevel.

Ability andexperience of consistently achieving performance objectives in theprovision of specialist homelessnesscasework.

Ability tocontribute to the identification dissemination and embedding ofexisting and emerging best practice within the team and across thewider Housing AdvisoryService.

Ability toeffectively communicate with a range of stakeholders includingapplicants members partners and soon.

Significantknowledge of welfare homelessness and housing policy strategieslegislation and case law and its implementation within a statutoryhomelessnessservice.

Significantknowledge of safeguarding best practice including policy strategieslegislation and case law and its implementation within a statutoryhomelessness service.
