Specialist Resettlement Coordinator

Brackenberry Ltd is seeking a highly skilled Specialist Resettlement Coordinator to join our team. As a Specialist Resettlement Coordinator, you will play a critical role in delivering a proactive person-centred outcome-focused and end-to-end case management approach to ensure our duties under the Homelessness Reduction Act are met.
Key Responsibilities:- Comprehensive Assessments: Undertake comprehensive assessments that lead to focused and coproduced Personal Housing Plans (PHP) for each applicant.
- Homelessness Reduction Act: Assess applicants under the prevention and relief duties, issue S.184 final decision letters, and ensure effective communication and management of expectations with customers.
- Risk Management: As appropriate, undertake the assessment and completion of high-quality risk management plans for applicants, utilising information from a range of stakeholders and the applicant themselves.
- Safeguarding: Minimise risk by identifying, reporting, and following up any safeguarding concerns.
- Safeguarding Knowledge: Significant knowledge of safeguarding best practice, including policy strategies, legislation, and case law, and its implementation within a statutory homelessness service.
- Homelessness Experience: Significant experience of providing specialist homelessness casework to single homeless applicants with more complex needs, such as applicants leaving institutions, including prisons and hospitals.
- Collaboration: Significant experience of working collaboratively to improve outcomes for single homeless applicants with more complex needs, such as applicants leaving institutions.
- Customer Service: Ability and experience to consistently deliver high levels of customer service to single homeless applicants with more complex needs.
- Legislation and Policy: Ability and experience of ensuring all aspects of casework are carried out in accordance with legislation, strategies, policies, and initiatives set out at the national, regional, and local level.
- Performance Objectives: Ability and experience of consistently achieving performance objectives in the provision of specialist homelessness casework.
- Best Practice: Ability to contribute to the identification, dissemination, and embedding of existing and emerging best practice within the team and across the wider Housing Advisory Service.
- Communication: Ability to effectively communicate with a range of stakeholders, including applicants, members, partners, and soon.
- Welfare and Homelessness Policy: Significant knowledge of welfare, homelessness, and housing policy strategies, legislation, and case law, and its implementation within a statutory homelessness service.
- You should be available to work immediately or at short notice.
- You should have the right to work in the U.K.
- This role requires an enhanced DBS.
