As possibly a newly qualified practitioner we will support you by helping you build your confidence and ability. We know you may be feeling anxious about having just qualified and we will support you in our caring environment.

We will provide training, shadowing opportunities and support you to work autonomously in the community and use your own initiative. The successful candidates will have sound clinical skills, be able to coordinate complex care packages, have good administration and IT skills, as well as a good working knowledge of CPA processes, report writing, organisational skills, safeguarding knowledge and the ability to identify issues and demonstrate ownership of change processes.

**UK driving licence with access to a vehicle for work is essential**

Main duties of the job

The CMHT is a lively, dynamic, and busy multi-disciplinary team which provides care coordination and recovery focused care for individuals with complex and challenging mental health needs.

You will need excellent communication and care skills. In addition, you will carry a caseload suitable for a band 5. As a member of the team, you will need to work alongside other agencies to promote holistic recovery focused care as well as producing reports, completing audits and running clinics as necessary.

The role purpose is:

*To be an active member of the multi-disciplinary team supporting the care planning for our service users.

*To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

*To undertake ongoing review, treatment and evaluation for service users known to the service

*To support the delivery of therapeutic interventions either individually or within a group setting.

*Support the transformation of the team to deliver local evidence based care

The transformation will happen in stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning across in May 2024. The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
