Main duties of the job
You will:
Be a qualified Health Visitoror qualifying in the next 3 months.
Provide supportive leadershipwithin the 0-19 team
Be an autonomous practitionerwho is able to support the needs of the team and service.
Take part in servicedevelopment, promote innovation and demonstrate skills in public health.
Have a flexible andconscientious approach to tasks.
Be able to communicateeffectively with partner agencies, promoting the work and reputation of theservice, working for the best interests of the child, and adopting a thinkfamily approach.
Asa key professional in the protection of children, you will be required to workcollaboratively with other agencies to meet their needs.

Job responsibilities
Duties and Responsibilities:
Public Health
To assess health needs at thelevel of the individual, community group and
population in accordance with local and national priorities.
Tocontribute to the development of public health strategies, needs
assessment/ profiling and local policies and procedures as appropriate.
Topromote the development of local health improvement networks.
The Health Visitor as an Administrative Manager isresponsible for:
Tomanage a defined caseload of clients, working within Bridgewater and
local servicespecific policy and guidelines and ensure Key Performance
Indicatorsare achieved.
To assume therole of Lead Professional where appropriate with the aim of
meetinghealth needs and to the co-ordination of support services as
appropriate.
To ensure theHealthy Child programme is effectively implemented in line
with policyand delivered to the caseload population.
To assess thehealth needs of children and their families, plan appropriate
healthimprovement interventions with individual families and evaluate
outcomes.
To develophealth plans with individuals and families based on shared
assessment ofhealth need, identifying in partnership, the requirement for
future HealthVisiting support.
To offerdirection, support and supervision to skill mix team colleagues in
relation todelegated case work
To act uponappropriate referrals from other members of the Primary Health
care team orother agencies and delegate work accordingly.
To identifythose children in need of protection, initiating Child Protection
Referral whenindicated and acting in accordance with Trust and Warrington
LocalSafeguarding Board policy and procedures.
Toparticipate in any proceedings required with regard to Trust Safeguarding
Policy andprocedure , Case Conferences, Core Group and Child in Need.
To completeCourt Reports as requested and adhere to time scales mandated
by Court andattend/participate in any subsequent Court proceedings.
Complete PDRsand return to work pro-forma as required to the designated
named staffwithin the team.
To support theTeam leader with delegated responsibilities in their absence
Clinical
Maintainactive status on NMC register acting in accordance with the
professionalcode and guiding documents in adherence with revalidation
framework.
To deliver theHealthy Child Programme using evidence-based practice to
activelypromote the health of children and young people.
To ensure thatHealth Visiting interventions are clinically effective, and
evidencebased.
To provideinformation, advice and guidance on the management of health
issues in arange of appropriate settings.
To assess theneed to follow up children accessing A+E department and Walk
in Centre,taking appropriate action when necessary.
To identifythose children whose health requires referral on for more
specialisedprofessional intervention taking the necessary action to progress
this.
To developevidence-based interventions/care programmes on a range of
subjects, foruse with clients.
To identify atan early age, those children requiring additional support and
interventionand to adhere to clinical pathways in supporting those children.
To promote theimmunisation of infants and pre-school children according to
Trust policyand in epidemic situations to the wider population as determined
by localneed.
To identifyminor childhood illness and as a Nurse Prescriber issue a
prescriptionfor treatment when indicated, appropriate and within the Health
Visitorsscope of practice.
To contributeto the development of health education literature for local
distribution,ensuring that clinical governance/quality assurance
requirementshave been met.
Leadership
To actas a positive and professional role model both within the service and
whenrepresenting the service to the wider community.
To lead andmanage change, encouraging others to recognise the need for
change,developing a shared vision and inspiring and motivating other team
members.
To participatein staff recruitment and selection when required.
Plan andprioritise with the Team Leader to meet the service objectives.
In partnershipwith the Team Leader and Practice Development Lead
contributetowards the development of evidence-based practice projects
ensuring thatclear standards are set, monitored and evaluated in line with the
0-19 servicekey performance indicators.
In conjunctionwith the Team Leader to undertake individual performance
reviews onnursing staff within Band 5, 4, 3 and 2.
Develop andimplement policies, procedures and guidelines relevant to own
area ofwork.
Team working

Responsibilities for data and information resources
Tosubmit statistical returns within a given period in order to fulfil data
requirementsand to contribute to ensuring adequate funding of the service.
To collect andmonitor local data , KPI data in order to improve outcomes
for children
Assess andanalyse local community / reach area data, in order to inform
workpriorities.
EnsureElectronic database of caseload is updated with current health
screeninginformation and levels of need
Training and Education
Toattend in service training as required in order to maintain current practice
To adhere tothe requirements of the NMC with regards to PREP.
To undertakeand complete mandatory training as required by Bridgewater
CommunityTrust.
To completeSafeguarding training as required and acquire and maintain a
knowledge ofcurrent legislation and guidance.
To identifypersonal training needs and take the necessary steps to meet
those needs.
Record Keeping/Records Management
Tocomplete Health Visitor records, in compliance with Bridgewater policy
and Localservice specific policy/procedure.
To maintainstrict confidentiality for all clients according to policy.
To maintainrecord keeping skills and knowledge by undertaking regular
training.
To adhere toBridgewater and local service guidance in relation to the
management ofRecords, including storage and transfer.
Supervision
The postholder will be able to recognise their own level of professional
competenceand identify sources of support as required.
To attend for1:1 supervision with the 0-19 Team Leader
To attend andpartake/engage in group clinical supervision.
To attend andpartake/engage in Safeguarding supervision .

Ensuring quality:
Participatesand engages with Personal Development Reviews
Maintains theup-to-date knowledge and skills required to safely undertake
the role,identifying any training needs and ensuring they are met.
Is conversantwith and adherent to care pathways used with the service.
Attendsrelevant in-service training and other professional seminars, lectures,
courses, andstudy days.
Ensure allmandatory training is accessed, kept up to date, recorded and
reported.
Undertakes therole of preceptor, mentor and trainer as required by the
service withregards to newly appointed staff and Student Nurses on
placement.
Assists in theeducation of learners.
Research and Audit
Toparticipate in research projects.
To be activelyinvolved with practice development in order to meet the needs
of theclient.
To ensurecurrent evidence base informs professional practice.
To comply withTrust requirements regarding data collection.
To participatein audit activity as required.
To ensureadherence to the Trusts clinical governance and nursing
strategies.
This job description is an outline of the role andresponsibilities. From time to time, due to the needs of the service, we mayask you to flexibly undertake other duties that are consistent with your roleand banding.
The 0-19 service is commissioned to deliver a service 8am-8pm.Therefore, the post holder would be expected to work flexibly providing aservice at evenings and weekends, dependant on the needs of the service.
As the 0-19 service is delivered across Warrington you willhave a base for administrative purposes only, as we may ask you to deliver aservice from a variety of bases dependent on needs of the service.
