Job summary

KEYROLE & RESPONSIBILITIES

1. Manage Health Interventions

The post holder will have the responsibility for theautonomous management of their own caseload of patients, providing acomprehensive, specialist nursing service for clients with often complex healthand social needs. The post holder will have responsibility for health specificclinical activity within outreach services which will include the monitoring ofstandards of care and service, staff support, continuous professional development,and supporting practice education.

You will work in partnership with GeneralPractitioners, Consultants, Nursing and Social Care colleagues to managepatients with a range of acute, non-acute and chronic mental health conditions.

Your role will include referring patients asappropriate to other members of the multidisciplinary team, secondary care andto other statutory and voluntary organisations. Providing and promotingevidence based care in line with appropriate clinical guidelines and workswithin clinical protocols, professional guidelines, codes of conduct and workswithin all relevant policies and procedural guidelines.

You will challenge professional and organisationalboundaries, identify areas for skill/knowledge development and apply these topractice to provide continuity and high quality patient health care for ourpatient group.

Your role will include communicating highly sensitivecondition-related information to patients, relatives and carers, as well asaudit outcomes of care against standards and initiates changes as necessary.

There will be a necessity to provide street outreachservices as part of this role which will involve providing services forpatients in locations outside of the surgery environment.

2. Promote Health

Proactively work with the community and otherstatutory and voluntary agencies empowering, enabling, encouraging andsupporting individuals, families and groups to address issues which affecttheir health and social well-being.

Initiates andcarry out programmes of health screening.

3 . Communication

Use developed communication, negotiation, conflictmanagement skills which require empathy, reassurance and promote informeddecision making to encourage good health outcomes for people who can experiencesignificant barriers to healthcare.

Keep accurate, contemporaneous documentation and careplans in accordance with the Nursing and Midwifery Council, local and nationalguidelines.

Develop effective communication links with otherprofessionals and agencies across all boundaries, at all levels of theorganisation including users, carers, social care and the voluntary sector toinsure effective delivery of care that provides the best outcome possible forservice users.

To listen and empathise with the needs of individualsinterfacing with the service adjusting communication styles accordingly. Workeffectively with interpreters.

4 . Training

To maintain competencies and professional developmentby regularly attending appropriate up dates and workshops to ensure a highstandard of quality care is delivered.

5 . Manage/LeadSelf/Others

Work collaboratively and in partnership with otherpractitioners on care practices, delivery and service development.

Plan and deliverown clinics.

Act as a role model for staff within the team andprovide effective leadership across the organisation.

Research and facilitate change in practice designed toimprove clinical outcomes and meet the needs of patients and care givers thatare consistent with local/national standards and current research.

Workcollaboratively as a team member to achieve the objectives of the team.

To recognise ethical and legal issues which haveimplications for nursing practice and clinical governance and contribute to theimplementation of systems to ensure clinical governance processes.

Partake inclinical supervision for own development.

Activelyparticipate in clinical and practice meetings.

Act as mentorfor nurses, HCAs and other professionals.

Lead on settingstandards and promotes best practice around mental health.

Act with honesty and integrity at all times and actsas a positive ambassador for Bevan Healthcare, understanding the CommunityBenefit Society model and ethos.

Undertake appropriatemandatory training.

Participate in networking and sharing practice eventswith other nurses within the locality.

Work within the guidelines and protocols of theNursing and Midwifery Council, foster an empowering attitude with colleagues.

Lead and manage projects as appropriate includingoutreach services.

6 . Educator

Provide expert advice on educational opportunitiesrelating to specialist knowledge and skills in clinical practice at anindividual, team and organisational level to facilitate and ensure thedevelopment and support of both standard and advanced nursing practice.

Assist with the recognition, assessment anddevelopment of the learning needs of colleagues, participating in the creation,delivery and evaluation of learning opportunities that fulfil theserequirements.
