Job summary
An exciting opportunity has arisen for an asthma practitioner with relevant experience of working with Children and Young People (CYP) with asthma, to contribute to improvements in CYP asthma care in Berkshire West.
Main duties of the job
The specialist Asthma Nurse Specialist will support the CYP Asthma Clinical Lead and the Senior Nursing Team to provide clinical advice and leadership in these respects:
o They will work closely with the CYP Clinical Leads for asthma to deliver the commitments set out in the National Bundle of Care for CYP with Asthma.
o They will provide a professional leadership role across the Trust, inclusive of clinical governance and a commitment to high-quality, evidenced based practice.
o They will help to ensure health colleagues treating CYP with asthma have access to the appropriate level of asthma training, contributing to the delivery of training and education.
o They will help to facilitate improvements in patient pathways, policies, clinical standards, and quality of care.
o They will contribute to embedding system wide policies, pathways and guidelines to reduce the over reliance of reliever medications and decrease the number of asthma attacks.
o They will take a lead role in the implementation of a risk stratification process to identify those CYP most at risk of asthma exacerbations.
They will help to decrease outpatient activity, emergency department activity and primary care activity through better control and access to more specialist input.

Job description
Job responsibilities
Working within the Royal Berkshire Hospitals Trust, the successful candidate will provide clinical advice and leadership to improve quality and outcomes for children and young people in line with the ambitions described in the NHSE CYP National Asthma Bundle of Care.
BOB ICB is supporting this 1year fixed term / secondment post with the aim of providing targeted interventions to patients in the Acute Trust, in order to prevent avoidable asthma attacks and escalations in care. This pilot will also complement the NHS Long Term Plan commitments to improve CYP asthma care and work towards meeting the CORE 20 Plus 5 ambitions to address the over reliance on reliever medications and decrease the number of asthma attacks.
The aim of this role is to support the ICB to achieve the NHSE/I ambition to:
Reduce avoidable harm from Asthma (control and reduce the risk of asthma attacks) Improve quality of life. Develop and embed a whole system approach to improve environment, education, personalised care, preventative medicine and accuracy/ timeliness of diagnosis. Person Specification
