Job summary

A key role within Forward Thinking Birmingham, we require Mental Health Nurses and Social Workers to work within our Referral Management Centre and play a key role in ensuring that high quality, clinically necessary care is being provided in the most appropriate and cost-effective therapeutic setting as part of Forward Thinking Birmingham's mental healthcare model (FTB).

We are seeking Mental Health Nurses, LD Nurses, but will consider applications from Children's Nurses and Social Workers who have direct mental health experience

Main duties of the job

You will be responsible, as part of the Clinical Mental Health Team within the Referral Management Centre which is currently operational from Monday-Friday 9-5, (however may be operational from 8am-8pm on weekdays depending on service need. The post holder will be managing referrals and referral enquiries. The post holder will be facilitating referral flow within the service for allotted appointments, provide clinical advice, undertake rapid clinical triage for cases requiring FTB Urgent Care Pathway and conduct the initial triage for the specialist services within FTB including Early Intervention referrals. You will be the point of contact for initial clinical queries, providing appropriate advice and guidance, and navigating the referrer/caller to the right care pathway or service (within or outside of FTB) in a timely and efficient manner with the aid of the 0-25 Service Directory and robust clinical processes and models in order to ensure the Referral Management Centre operates a streamline and effective service.

Person Specification

Qualifications

Essential

Qualification/current and active registration with the Health & Care Professions Council or equivalent ( NMC-registered RMN, graduate level training in mental health related subject, Social Worker, OT).

Desirable

Formal qualification relevant to role ( telephone triage training) Evidence of continuing post-registration/graduate development

Experience

Essential

Experience of working as a mental health practitioner with Children, Families and Young people between the ages of 0-25 Knowledge of mental health conditions, diagnoses and clinical risk Excellent communication/ interpersonal skills in order to deal effectively with complex and sensitive service user information, distressed or aggressive service users or carers or those with an impaired level of understanding

Desirable

Broad understanding and breadth of experience of the types of services used by service users relevant to the target population (, NHS, social care and voluntary and independent sector) Detailed experience working within a mental health setting

Professional Skills

Essential

Ability to self-organise/manage and deliver a high standard of clinical coordination and triage service

Desirable

Knowledge and understanding of clinical governance
