This broad, patient centred and high profile role is integral to the operational running of the department which is currently striving to meet the challenge of additional demand across the service. The main function of the role will be to coordinate and schedule the patient's Inpatient pathway, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the processes, utilising the consultants theatre capacity in line with the trusts KPI targets / Actively monitoring unfit patients.

Main duties of the job

The ideal candidate will have experience of working in the NHS. The post holder will have an increasing responsibility for the development, coordination and facilitation of the scheduling process, will ensure all patients are robustly tracked through the appropriate pathway and ensure that all the relevant data is accurately recorded, reported and analysed in a timely manner. Candidates should have excellent organisational and communications skills, be able to work under pressure whilst ensuring all deadlines are achieved.

Job description

Job responsibilities

To be responsible for the management of the patient pathway, from referral to treatment, through diagnostics, treatment planning and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process

To have a sound understanding of 18 week and cancer waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.

To provide a weekly update to Service Manager so they are aware of any pathway delays or potential breaches of waiting times targets.

To routinely attend performance review meetings and ensure that agreed actions and outcomes are requested, documented and monitored.

To monitor and work alongside the 6-4-2 trust policy ensuring best utilisation for the unit and trust

To be responsible for booking and sending out any relevant correspondence to patients as required

To liaise with consultants and their teams to monitor and ensure correct case mix on theatre lists, making appropriate staff aware of any changes, including Site/Bed manager and Theatre staff using the agreed protocols / shared files.

To ensure patient cancellations are allocated new TCI dates within the specified time limits.

To review / chase / update all Active monitoring patients weekly and to escalate any potential issues that may arise

Monitor elective surgery / imaging waiting times, proactively identifying risks, and taking corrective actions to ensure relevant targets are not breached, in conjunction with the Service teams, and clinical teams.

Deal with non-clinical queries from patients, their carers, general practitioners and members of the admin & clerical team.

Person Specification

Qualifications

Essential

Good level of general education, grade c or above in English language and maths GCSE equivalent.

Desirable

Educated to A level standard or NVQ in customer care EDCL

Experience

Essential

Experience of working within the NHS or a large organisation Knowledge and understanding of Government and Local Targets Understanding of data protection act and freedom of information act

Desirable

Knowledge of ICS/ Insight/ ICE/Noteon or similar systems in the NHS Good knowledge of the processes within a hospital setting

Skills and Abilities
