Main duties of the job
For Advanced Nurse Practitioner:
The successful candidate will practiceautonomously, within their speciality and be able to provide patient-centredcare. This will encompass the skills of clinical assessment, examination, diagnosis,and treatment within an agreed scope of practice.
Be involved in decision-making and support newways of working that create efficient and patient focused services.
Support the ongoingdevelopment of clinical practice and standards of care within the service,including the development of policies, procedures, protocols, and guidelines incollaboration with multi-disciplinary colleagues.
Administration requirements to deliver safepatient care and be involved in the continuity of care.
Involvement in signposting, triage, and seeingpatients.
Demonstrate a high level of expertise, providing advice, education, and support to patients, their families, and their carers.
For the Advanced Pharmacist:
An enthusiastic and forward-thinking clinical pharmacist.
Develop and manage all aspects of medicines across the practice network.
Working with highly skilled multi-disciplinary team you will work in a patient facing role based in both of our practice sites. This is an evolving role with huge scope for development.

Thornaby and Barwick Medical Group is part of BYTES PCN and provide services to patients via a multi-disciplinary team of GPs, Advanced Practitioner, Nurses, HCAs, GPAs, Physician Associate, Pharmacy Technician, MSK, Social Prescribers, Adult and children mental health professionals, alongside competent and efficient admin teams and managers.
We are a training practice and regularly host GP Trainees and hold education meetings to develop shared learning.

The Advanced Clinical Practitioner (ACP) hasbeen assessed and deemed as competent in practice using their expert clinicalknowledge and skills. They have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis, and treatment of patients.
The post holder works at the level of advancedclinical practice and has developed and can evidence the underpinningcompetencies applicable to the specialty or subject area, , the knowledge, skills,and behaviours appropriate to the healthcare professionals (HCP) setting andjob role.
The role provides an opportunity for patients toavoid the need to be referred to a GP, thereby enabling them to receive timelycare and negate unnecessary delay in receiving treatment.
An ACP is autonomous in making decisions basedon assessment, diagnosis and interpretation of test results and will initiate, evaluate,and modify a range of interventions which may include prescribing medicines,therapies, lifestyle advice and care.
The following are the core responsibilities of theAdvanced Clinical Practitioner.
There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffing levels,however core responsibilities include, but are not limited to:

Provide chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequired
Maintain accurate clinical records inconjunction with extant legislation and to ensure SNOMED CT codes are usedeffectively
Critically appraise and synthesise the outcomeof relevant research, evaluation and audit, using the results to underpin ownpractice and to inform that of others
Contribute to practice targets (QOF, etc.), complying with local andregional guidance
Chaperone patients where necessary
Support the team in dealing with clinicalemergencies
Support the team with all safeguarding matters inaccordance with local and national policies
Understand practice and local policies forsubstance abuse and addictive behaviour, referring patients appropriately
Deliver opportunistic health promotion whereappropriate
Secondary responsibilities
In addition to the primary responsibilities, the AdvancedClinical Practitioner may be requested to:
Support practice and PCN leads to ensurecompliance, and undertaking audits as necessary
Monitor and ensure the safe storage, rotation,and disposal of medicines
Participate in local initiatives to enhanceservice delivery and patient care
Support and participate in shared learningwithin the organisation
Continually review clinical practices,responding to national policies and initiatives where appropriate
Participate in the review of significant andnear-miss events applying a structured approach , root cause analysis (RCA)
Commensurate with their role, support thedevelopment of clinical services within the organisation, liaising withexternal agencies and professional organisations as required
