Main duties of the job

For Advanced Nurse Practitioner:

Be involved in decision-making and support newways of working that create efficient and patient focused services.

Administration requirements to deliver safepatient care and be involved in the continuity of care.

Develop and manage all aspects of medicines across the practice network.

Thornaby and Barwick Medical Group is part of BYTES PCN and provide services to patients via a multi-disciplinary team of GPs, Advanced Practitioner, Nurses, HCAs, GPAs, Physician Associate, Pharmacy Technician, MSK, Social Prescribers, Adult and children mental health professionals, alongside competent and efficient admin teams and managers.

The Advanced Clinical Practitioner (ACP) hasbeen assessed and deemed as competent in practice using their expert clinicalknowledge and skills. They have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis, and treatment of patients.

The post holder works at the level of advancedclinical practice and has developed and can evidence the underpinningcompetencies applicable to the specialty or subject area, , the knowledge, skills,and behaviours appropriate to the healthcare professionals (HCP) setting andjob role.

The role provides an opportunity for patients toavoid the need to be referred to a GP, thereby enabling them to receive timelycare and negate unnecessary delay in receiving treatment.

An ACP is autonomous in making decisions basedon assessment, diagnosis and interpretation of test results and will initiate, evaluate,and modify a range of interventions which may include prescribing medicines,therapies, lifestyle advice and care.

The following are the core responsibilities of theAdvanced Clinical Practitioner.

There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffing levels,however core responsibilities include, but are not limited to:

Critically appraise and synthesise the outcomeof relevant research, evaluation and audit, using the results to underpin ownpractice and to inform that of others

Contribute to practice targets (QOF, etc.), complying with local andregional guidance

Chaperone patients where necessary

Support the team in dealing with clinicalemergencies

Support the team with all safeguarding matters inaccordance with local and national policies

Secondary responsibilities

Support practice and PCN leads to ensurecompliance, and undertaking audits as necessary

Monitor and ensure the safe storage, rotation,and disposal of medicines

Participate in local initiatives to enhanceservice delivery and patient care

Support and participate in shared learningwithin the organisation

Continually review clinical practices,responding to national policies and initiatives where appropriate

Participate in the review of significant andnear-miss events applying a structured approach , root cause analysis (RCA)
