Job summary

Dr Mahbub's Surgery are looking for an experienced, motivated Advanced Nurse Practitioner to work alongside our small but friendly team. The role is for 12 hours per week over 3 days (Monday to Wednesday).

Main duties of the job

The Advanced Nurse Practitioner (ANP) has been assessed as competent inpractice using their expert clinical knowledge and skills. They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.

The ANP provides an opportunity for patients to receive timely care andnegate unnecessarily delay in receiving treatment and without the need to referto a GP.

ANPs in primary care can develop close, long-term relationships withtheir patients and work in partnership with them to achieve optimum health.They are autonomous in making decisions based on assessment, diagnosis andinterpretation of test results.

They can independently prescribe appropriate medication, evaluate orrefer to other specialists if necessary.

About us

Dr Mahbub's Surgery at Brace Street Health Centre, Walsall is a small, well established and friendly GP practice serving a patient population of approximately 3,300. We are looking for a dedicated and experienced Advance Nurse Practitioner to join our successful candidate will be responsible for providing a full range of primary care services to our mostly BAME patient population.

Job description

Job responsibilities

Working alongside the doctors and nurses at the surgery, the post holder will be responsible for the following:

Clinical

1. To see patients independently. Todiagnose and treat within the setting of the GP facility.

2. Assist in the development of anintegrated approach to the delivery of patient care involving all relevantstakeholders including health and social care organisations, particularlywithin the primary care field.

3. To provide telephone assessment topatients calling the surgery and inviting them to attend the surgery ifappropriate.

4. To provide clinical expertise topatients walking in to, or with appointment at the surgery, by assessing andtreating their medical condition.

5. To provide clinical support tocolleagues at the surgery.

6. Continue to develop, expand, cascadeand role model own personal clinical expertise as autonomous/independentpractitioner.

7. In partnership with others,challenge and critically evaluate the boundaries of autonomous practice, suchthat patient access, choice and outcomes will improve.

8. Help in the development andevaluation of clinical guidelines. Involved in team discussions to ensure bestpractice.

9. Develop own areas of specificclinical expertise in order to link acute, primary, community and emergencycare sectors in expanding nursing practice within the surgery.

Organisational

1. To take clinical responsibility forthemselves and to provide assistance and supervision to other healthprofessionals as required.

2. To assist in the maintenance ofclinical standards and to share in the efficient running of the service.

3. To share the responsibility for thesecurity and care of equipment.

4. To be aware of the Political andSocial issues which affect the delivery of healthcare.

5. To help in the development of policyand procedure within the surgery.

Communication

1. To maintain accurate patient recordsfor users of the service, using the designated software system for thepractice.

2. To be aware of responsibilitiesunder the Data Protection Act and Caldicott principles.

3. To attend regular team meetings.

4. To ensure effective communicationwith colleagues and the wider health care community as appropriate when referringa patient.

5. Working with interpreters [linkworkers] for non-English speaking patients.

6. Record and report all clinicalincidents to GP Lead.

7. Assess individual communicationneeds and understanding for informed consent.

Education, Training & Research

1. Contribute to data gathering asneeded for QOF, enhanced services and KPI targets.

2. To take responsibility for ownprofessional development undertaking training for specific clinical tasks.

3. Participate in clinical audit. For ClinicalGovernance and Training purposes a random sample of the clinical notes writtenby every clinician (nurses and doctors) will be audited regularly against a setof criteria. These criteria include clarity and accuracy of documentation aswell as standards of good clinical practice.

4. Participate in research programmesas appropriate.

5. Participate in the development andimplementation of standards and audit activities.

General

The responsibilities of the post willbe the subject of on-going review between the post holder and line manager. Thepost holder may be required to undertake other duties to meet the needs of thisdeveloping service.

1. The Post Holder will be required towork flexibly, which may include additional hours, to cover holidays andsickness.

2. To maintain active NMC registration.

3. To adhere to Organisational policies.

4. To ensure confidentiality at alltimes, only releasing confidential information obtained during the course ofemployment to those acting in an official capacity in accordance with policiesand Caldicott guidelines.

5. Attend all mandatory trainingcourses and the updates and any study days as appropriate to the post

6. Attend performance and developmentreviews (PDR) with your line manager. You will have a PDR at least once a yearand twice during the probationary period with the line manager. This processwill include the development of a Personal Development Plan.

7. To carry out other dutiescommensurate with the grade as directed.

8. Attend meetings as requested by thepractice leads.

9. Establish and maintain effectiveworking relationships with co-workers, supervisors and the general public.

10. Maintaining regular consistent andprofessional attendance, punctuality, personal appearance and adherence to relevanthealth and safety / environmental procedures.

11. To have a good understanding of Practice policies and procedures.

Person Specification

Qualifications

Essential

- Registered Nurse with Nursing and Midwifery Council. - Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice. - Post graduate diploma or degree for Advanced Practice Qualification up to December 2020. - Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) route. - Qualified Independent Nurse Prescriber on the NMC register.

Experience

Essential

- Experience of practice within the four pillars of clinical practice; leadership and management, education and research with demonstration of core capabilities and area specific clinical competencies. - Job plan that demonstrates advanced nursing practice and has equity with peers working at this level. - Experience of prescribing and undertaking medication reviews. - Experience of working in a primary care environment.

Desirable

- Qualified triage nurse. - Minor illness qualification. - Teaching qualification.
